Coverage Criteria for Yorvipath (palopegteriparatide)
Initial Therapy and Continuation Therapy
Approve for 1 year if the patient meets ONE of the following (A or B):
A) Initial Therapy
- A.i: Patient cannot be well-controlled on calcium supplements and active forms of vitamin D according to the prescriber
- A.ii: Patient has sufficient 25-hydroxyvitamin D stores (baseline before initiating Yorvipath) according to the prescriber
