Patient lift criteria: transfer between bed and a chair, wheelchair or commode requires the assistance of more than one person

Items are considered medically necessary when the criteria in the applicable policy statements are met for the specific item.

General: Coverage requires that the device billed corresponds to the HCPCS code and meets the policy's medical necessity criteria as referenced; see Medicare NCD/LCDs where applicable.

Coverage for seat lift mechanisms, patient lifts, multi‑positional transfer systems, combination transfer/mobility devices, and standing devices may vary by member plan. While many of these items are covered when the specific medical necessity criteria in this policy are met (see individual device sections), some plans explicitly exclude certain lift-related items as convenience or self‑help. Providers should confirm member plan benefits and prior authorization requirements before ordering or billing. Documentation submitted must support that the requested device meets the applicable policy criteria and is included in the treating physician’s plan of care.

The following specific items are considered not medically necessary across Cigna‑administered plans: seat lift mechanisms placed over or on top of a toilet (HCPCS E0172), bathroom or toilet patient lifts not otherwise classified (HCPCS E0625), and fixed patient lift systems (HCPCS E0640). In addition, bathroom/toilet lifts and other lifts that are primarily intended for toileting or bathing assistance are treated as self‑help/convenience items and are generally not covered.

Requests for duplicate equipment are generally considered a convenience and therefore not medically necessary. Examples include providing additional bathroom/toilet lifts, ceiling lifts that require attachment to real property, or other redundant lift systems for the same member. Replacement of a medically necessary device is allowed only for anatomical change or when reasonable wear and tear renders the item nonfunctional and not repairable, and only when the plan covers the specific item.

The following HCPCS codes are explicitly listed as not medically necessary and will be denied if billed as covered items: E0172, E0625, and E0640. Providers should not submit claims for these codes as covered DME under Cigna‑administered plans.
