Professional staff-assisted home hemodialysis - Medical Necessity
Professional staff-assisted home hemodialysis (CPT 99512) is considered medically necessary when ALL of the following are met:

The policy notes that the presence of a professional (e.g., nurse) during home dialysis is generally regarded as a convenience and not medically necessary. Only in rare, specific situations—where the patient meets the policy's defined clinical exceptions—would staff-assisted home hemodialysis be considered medically necessary.
Staff-assisted home hemodialysis is covered only when it meets ALL of the medical necessity criteria specified in the policy. If services do not satisfy every required criterion, they are considered not medically necessary and will not be covered.

CPT 99512 (Home visit for hemodialysis) will be covered only when documentation shows that ALL five medical necessity criteria in the policy are met: stability on hemodialysis, a well-functioning vascular access, a medical contraindication to leaving home, inability/failure of the patient or non‑professional caregiver to perform home hemodialysis after adequate training, and documented refusal by local hemodialysis centers to accept the patient for in‑facility hemodialysis.
- Record and submit clinical documentation addressing each of the five required criteria listed in the policy.
- Include CPT 99512 on claims only when all five criteria are clearly documented in the medical record.
Provider must follow Coverage Policy and member’s benefit plan terms
Follow the policy instructions in the Coverage Policy and Instructions for Use when preparing medical necessity requests and claims; ensure documentation references the applicable benefit plan terms and any delegated vendor guidance if used.

- Medical directors may exercise clinical judgment for individual determinations; provide supporting documentation as requested.
Claims must use codes effective on date of service; non‑covered codes are not payable
Submit claims using the most appropriate codes that were in effect on the date of service; services not accompanied by covered code(s) under the applicable Coverage Policy will be denied.
- Do not bill deleted or not‑effective codes for the date of service; such codes may be ineligible for reimbursement.
- Include CPT 99512 only when it is an active, covered code for the service date and medical necessity criteria are met.
