Coverage Criteria for Opzelura

Initial therapy — Atopic dermatitis

Covered when ALL of the following are met

Atopic Dermatitis approval criteria: A. Patient is ≥ 2 years of age; AND B. According to the prescriber, the patient has mild to moderate atopic dermatitis; AND C. Patient has atopic dermatitis involvement estimated to affect ≤ 20% of the body surface area; AND D. BOTH (i and ii): i. ALL: (a) Patient has tried at least one medium-, medium-high, high, and/or super-high-potency prescription topical corticosteroid; (b) That topical corticosteroid was applied daily for at least 28 consecutive days; (c) According to the prescriber, inadequate efficacy was demonstrated with that topical corticosteroid; AND ii. ALL: (a) Patient has tried at least one topical calcineurin inhibitor (e.g., tacrolimus ointment, pimecrolimus cream); (b) That topical calcineurin inhibitor was applied for at least 28 consecutive days; (c) According to the prescriber, inadequate efficacy was demonstrated with that topical calcineurin inhibitor; AND E. The medication is prescribed by, or in consultation with, an allergist, immunologist, or dermatologist.8 weeks

Likewise, concurrent use with other potent systemic immunosuppressants (for example, azathioprine or cyclosporine) is not recommended and may trigger denial because safety and efficacy of combination use have not been established. The prior policy allowed an exception to the prescription topical corticosteroid trial when lesions involved sensitive areas (face, eyelids, skin folds, and/or genitalia); that exception was removed in the current revision. The update requires documentation of the specified topical corticosteroid trial duration regardless of lesion location, so requests for coverage must show the trial even when disease affects sensitive areas.

Opzelura is considered not medically necessary for any use other than the FDA‑approved indications (atopic dermatitis and nonsegmental vitiligo). The policy explicitly lists noncovered uses including treatment of alopecia and plaque psoriasis and emphasizes that requests for indications outside the approved labeling will be denied.

The policy's "Conditions Not Covered" section was updated.
