Coverage Criteria

inv-01: ETS — Initial Therapy Criteria

Endoscopic thoracic sympathectomy (ETS) for primary palmar and axillary hyperhidrosis is considered medically necessary when BOTH of the following are met:

ETS medical necessity: EITHER: (a) medical complications secondary to hyperhidrosis (e.g., skin maceration with secondary infection) OR (b) significant impact on age-appropriate activities of daily living as a result of hyperhidrosis; AND failure of at least two nonsurgical treatments (e.g., prescription topical agent, prescription oral medication, onabotulinumtoxinA injection, or iontophoresis).

inv-02: Axillary sweat gland surgery — Initial Therapy Criteria

Surgical removal of axillary sweat glands (including use of curettage and liposuction) for primary axillary hyperhidrosis is considered medically necessary when ALL of the following are met:

Axillary gland surgery medical necessity: All ETS medical necessity criteria have been met (i.e., EITHER medical complications from hyperhidrosis OR significant impact on age-appropriate activities of daily living) AND failure of at least two nonsurgical treatments. Includes curettage and liposuction methods as acceptable when the above criteria are documented.

Excision, curettage, or combined limited excision with curettage are described approaches; complete radical excision is generally not recommended due to scarring and range-of-motion concerns.

inv-03: Endoscopic Thoracic Sympathectomy — Covered Indications

Endoscopic Thoracic Sympathectomy (ETS) covered when ALL of the following are met:

ETS coverage: Procedure is for primary palmar or axillary hyperhidrosis and applicable policy medical necessity criteria are met (i.e., documented complications or significant ADL impact and failure of ≥2 nonsurgical treatments).

ETS is considered not medically necessary for craniofacial and/or secondary hyperhidrosis and for repeat or reversal procedures.

inv-04: Surgical Removal of Axillary Sweat Glands — Covered Indications

Surgical removal of axillary sweat glands covered when ALL of the following are met:

Axillary gland removal coverage: Procedure is for primary axillary hyperhidrosis and applicable policy medical necessity criteria are met (i.e., documented complications or significant ADL impact and failure of ≥2 nonsurgical treatments). Curettage (with or without tumescent suction) or excision techniques are acceptable when criteria are met.

Suction-assisted lipectomy (liposuction) when used as the sole method to remove axillary sweat glands is considered not medically necessary; use appropriate CPT codes for excision or curettage-based procedures.

Nonprescription and over-the-counter (OTC) drugs for hyperhidrosis are not covered under many medical benefit plans. Refer providers and members to the applicable pharmacy benefit to determine coverage availability and specific terms and conditions.

Surgical treatment of secondary hyperhidrosis is considered not medically necessary because appropriate management should target the underlying condition (for example, hyperthyroidism, diabetes mellitus, or hyperpituitarism) rather than definitive surgical procedures for sweating.

Endoscopic thoracic sympathectomy (ETS) is explicitly considered not medically necessary for craniofacial and/or secondary hyperhidrosis, and ETS performed as a repeat or for reversal of a prior sympathectomy is also considered not medically necessary. The literature review indicates there is insufficient evidence that reversal or repeated sympathectomy safely and effectively addresses compensatory sweating or other ETS complications; providers should document that cases do not meet covered indications before proceeding.

The policy lists specific procedures considered not medically necessary, including liposuction as the sole method of removing axillary sweat glands, and repeat or reversal of endoscopic thoracic sympathectomy (ETS). The policy notes there is limited evidence supporting reversal or repeated sympathectomy as safe and effective; accordingly, these indications are not covered.

Sympathectomy for plantar (sole) hyperhidrosis has been removed from the policy's covered indications and ETS is not recommended for sweaty feet. Providers should note that ETS for plantar hyperhidrosis is not a covered indication and that more conservative therapies are recommended for plantar symptoms before considering invasive approaches.

Prior Authorization Required

Prior authorization is required for Endoscopic Thoracic Sympathectomy (ETS) and for surgical removal of axillary sweat glands. Submit a prior authorization request with complete clinical documentation demonstrating that the policy medical necessity criteria are met before performing these procedures.

Procedures requiring prior authorization: ETS (CPT 32664); surgical removal of axillary sweat glands (CPT 11450, 11451, 15877, 15878, 17999).
Do not submit requests for procedures intended for craniofacial or secondary hyperhidrosis, repeat/reversal of ETS, or liposuction as the sole method of axillary sweat gland removal (these are not medically necessary).

Provide supporting documentation that demonstrates the medical necessity of the requested surgical procedure per the policy criteria. Include detailed clinical notes, records of prior treatments and responses, description of functional impact (age‑appropriate activities of daily living), and any evidence of medical complications secondary to hyperhidrosis.

Hyperhidrosis is defined as excessive sweating beyond thermoregulatory needs and is classified as either primary focal (idiopathic), which typically affects the palms, axillae, or soles and is caused by an overactive sympathetic nervous system, or secondary, which is generalized and due to an underlying medical condition (for example, malignancy, infection, spinal cord injury, neurologic, or endocrine disorders). Primary focal hyperhidrosis can impair activities of daily living and is the form addressed by the policy's surgical coverage criteria.
