B: Medication is prescribed by or in consultation with a professional experienced in the use of chelation therapy (e.g., a medical toxicologist or a poison control center specialist)

Concomitant use of Chemet with other parenteral chelating agents (for example, calcium disodium versenate [CaNa2EDTA] or dimercaprol [BAL]) is not recommended because data on combined use are not available. If a patient has received CaNa2EDTA with or without BAL, Chemet may be used for subsequent treatment only after a waiting interval of 4 weeks.

Chemet is considered not medically necessary for chronic exposures and for chelation intended to treat chronic medical or psychiatric conditions because evidence does not support benefit and there is potential for harm.

Excluded uses include (but may not be limited to):

• Chelation to treat chronic medical or psychiatric conditions (examples cited in the literature include treatment of intermittent claudication, management of symptoms of autism, or prevention/management of neurodegenerative diseases such as Alzheimer's disease or Parkinson's disease).

• Chronic arsenic exposure — trials and case reports have not demonstrated clinical or biochemical benefit of DMSA for chronic arsenicosis.

• Chronic mercury exposure — although chelation may increase metal excretion, therapeutic efficacy in reducing morbidity or mortality from chronic inorganic mercury exposure is unestablished.

Other uses that are considered not covered include prophylactic administration of Chemet (for example, use to prevent lead poisoning in a lead-containing environment) and other chronic or off‑label indications where benefit has not been established.

Examples of additional non‑covered uses include treatment of chronic conditions or symptoms allegedly due to dental amalgams, long‑term/off‑label courses for non‑acute exposures, and any uses not specifically listed as covered in this policy.

Background on Chemet (DMSA)

Chemet (meso‑2,3‑dimercaptosuccinic acid; DMSA) is an oral heavy‑metal chelating agent formulated as succimer capsules. It is indicated for treatment of pediatric lead poisoning in patients aged ≥ 1 year with elevated blood lead levels (policy aligns with the FDA indication and supporting clinical literature).
