Statins Step Therapy Policy product(s) are covered as medically necessary when the step therapy criteria are met. Any other exception is considered not medically necessary unless covered by preventive services mandates.

Statins Step Therapy Policy product(s) are covered as medically necessary only when the step therapy criteria in this policy are met. Any request that does not meet the step therapy criteria is considered not medically necessary, except where coverage is required by the Affordable Care Act/HRSA/Public Health Service Act section 2713 for preventive services (see preventive coverage exception below).

Requests for a Step 2 product must include documentation that the patient has tried one Step 1 product prior to approval. If such documentation is not provided, the request will be considered not medically necessary unless an established exception applies (for example, documented inability to swallow tablets/capsules or the ACA/HRSA preventive services exception).

This Step Therapy rule is enforced at the point of service. If the Step Therapy requirement for a Step 2 product is not met at the time of service, coverage will be determined by the Step Therapy criteria in this policy. All approvals are provided for a duration of 1 year.

Documentation Required

Document prior Step 1 product use

Providers must document the patient’s prior use of a Step 1 product when requesting approval for a Step 2 product. Documentation should include the product name, dates of therapy, and the clinical reason the Step 1 product was discontinued or deemed inappropriate (for example, intolerance or contraindication). When the patient cannot swallow tablets or capsules, document the swallowing difficulty to support use of the specified alternate products (Atorvaliq, Flolipid, or Ezallor Sprinkle).

Background and Clinical Context

Statins (HMG-CoA reductase inhibitors) are the first-line lipid-lowering agents used for primary and secondary prevention of atherosclerotic cardiovascular disease (ASCVD). They are indicated as an adjunct to diet to lower total and LDL cholesterol and reduce cardiovascular risk. Clinical guidelines recommend maximally tolerated statin therapy and optimization of LDL-C before adding non‑statin agents; intensity selection (high, moderate, or low) depends on ASCVD risk, comorbidities, and tolerability.
