Coverage and Medical Necessity Criteria

Pradaxa capsules — Medical Necessity

Pradaxa capsules are considered medically necessary when ALL of the following are met (1 and 2):

Initial conditions

FDA-approved indications (A–D): A. Atrial fibrillation (or atrial flutter) — Approve for 1 year if patient ≥ 18 years of age; B. Deep vein thrombosis or pulmonary embolism, Treatment — Approve for 1 year if patient ≥ 8 years of age; C. Deep vein thrombosis or pulmonary embolism, To reduce the risk of recurrence — Approve for 1 year if patient ≥ 8 years of age; D. Deep vein thrombosis or pulmonary embolism in a patient undergoing hip replacement surgery, prophylaxis — Approve for 60 days if patient ≥ 18 years of age.
Other uses with supportive evidence (E–F): E. Deep vein thrombosis in a patient undergoing knee replacement surgery, prophylaxis — Approve for 60 days if patient ≥ 18 years of age; F. Treatment or prevention of other thromboembolic-related conditions — Approve for 6 months if patient ≥ 8 years of age AND patient meets one of (a) or (b): (a) failure, contraindication, or intolerance to warfarin, fondaparinux, or a low molecular weight heparin product (e.g., enoxaparin, dalteparin); OR (b) patient has been started on dabigatran capsules for an acute thromboembolic condition. Note: a patient who has tried apixaban (including oral suspension/sprinkle), rivaroxaban (including oral suspension), or edoxaban is not required to try warfarin, fondaparinux, or LMWH.
Examples of other conditions: superficial vein thrombosis, splanchnic vein thrombosis, hepatic vein thrombosis, prophylaxis in high-risk patients

Pradaxa oral pellets — Medical Necessity

Pradaxa oral pellets are considered medically necessary when ALL of the following are met (1 and 2):

FDA-approved indications (A–B): A. Venous thromboembolic events, Treatment — Approve for 1 year if patient is ≥ 3 months to < 12 years of age (examples include DVT, cerebral venous thrombosis, pulmonary embolism, central venous thrombosis); B. Venous thromboembolic events, To reduce the risk of recurrence — Approve for 1 year if patient is ≥ 3 months to < 12 years of age.
Other uses with supportive evidence (C): C. Treatment or prevention of other thromboembolic-related conditions — Approve for 6 months if patient is ≥ 3 months to < 12 years of age AND patient meets one of (a) or (b): (a) failure, contraindication, or intolerance to ONE of: warfarin, fondaparinux, or a low molecular weight heparin product (e.g., enoxaparin, dalteparin); OR (b) patient has been started on Pradaxa oral pellets for the treatment of an acute thromboembolic condition.
