Covered when ANY of the following are met:

Primary Prevention of SCD: EP study to determine eligibility for an ICD in individuals with ischemic heart disease; to determine eligibility for an ICD in an individual with NSVT due to prior MI >40 days and LVEF 36–40%; in individuals with CAD after acute MI to determine ICD eligibility with NSVT 4–40 days post-MI when either revascularization was performed at time of event with LVEF ≤40% OR obstructive CAD not revascularized and not amenable to revascularization.

Secondary Prevention of SCD: EP study for risk stratification in NICM in individuals who experience syncope presumed due to ventricular arrhythmia and who do not meet primary prevention ICD indications; to determine eligibility for an ICD after VF or hemodynamically unstable VT during an acute MI (<48 hours) with NSVT ≥4 days after revascularization; to determine ICD eligibility in ischemic heart disease with unexplained syncope who do not otherwise meet EF-based ICD criteria.
