Coverage Criteria for Transcatheter Cardiac Ablation

Transcatheter ablation for supraventricular tachycardia (SVT) — adults

Covered when ANY of the following arrhythmias are present in an adult ≥18 years:

SVT indications in adults: Accessory pathway ablation for AVRT, preexcited atrial fibrillation, or high-risk EP study findings; cavotricuspid isthmus (CTI)–dependent atrial flutter; CTI-dependent atrial flutter induced at the time of AF ablation; junctional tachycardia when medical therapy is not effective or is contraindicated; recurrent symptomatic non-CTI-dependent atrial flutter; symptomatic AVNRT; symptomatic focal atrial tachycardia as an alternative to pharmacological therapy.

CPT 93653 applies.

Transcatheter ablation for supraventricular tachycardia (SVT) — children

Covered in children <18 years of age

Pediatric SVT: Transcatheter ablation (CPT 93653) is considered medically necessary in children <18 years of age.

Per PACES/HRS guidance: generally after failed pharmacotherapy or intolerable medication side effects; in larger children/adolescents may be offered as definitive therapy to avoid chronic medication.

Transcatheter ablation for ventricular arrhythmias and PVCs — adults

Covered when ANY of the following are met in an adult ≥18 years:

PVC and ventricular arrhythmia indications: Premature ventricular contractions (PVCs) when EITHER PVC burden is >15% of total beats OR PVCs are symptomatic and medical/pharmacologic management is not effective, is contraindicated, is not tolerated, or is not preferred by the individual; ventricular arrhythmias (ventricular tachycardia or ventricular fibrillation) when medical/pharmacologic management is not effective, is contraindicated, is not tolerated, or is not preferred by the individual.PVC burden >15%

Explicit NMN conditions: Inappropriate sinus tachycardia (IST); Premature atrial contraction (PAC).

These indications are considered not medically necessary for transcatheter ablation.

Ongoing management — AVNRT

Covered or reasonable options for ongoing management of AVNRT per guideline recommendations

First-line for ongoing management: Oral verapamil or diltiazem OR oral beta blockers are recommended for ongoing management in patients with AVNRT who are not candidates for, or prefer not to undergo, catheter ablation (Class I).

Level of Evidence B-R

Alternatives when first-line ineffective/contraindicated: Flecainide or propafenone reasonable in patients without structural or ischemic heart disease; oral sotalol or dofetilide may be reasonable; oral digoxin or amiodarone may be reasonable.

Recommendations for cavotricuspid isthmus (CTI)-dependent and non-CTI-dependent atrial flutter

CTI-dependent flutter: Catheter ablation of the CTI is useful in symptomatic or pharmacologically refractory CTI-dependent atrial flutter (Class I); CTI ablation is often preferred to long-term pharmacological therapy due to >90% single-procedure success and favorable safety profile.

Non-CTI-dependent flutter: Catheter ablation is useful in recurrent symptomatic non-CTI-dependent flutter after failure of at least 1 antiarrhythmic agent (Class I) and may be reasonable as primary therapy in selected patients (Class IIa).

Asymptomatic recurrent flutter: Catheter ablation may be reasonable for asymptomatic patients with recurrent atrial flutter (Class IIb).

Management — Asymptomatic pre-excitation (WPW)

When to consider ablation in asymptomatic patients with ventricular pre-excitation

High-risk EP features: Catheter ablation of the accessory pathway is reasonable in asymptomatic patients if an EP study identifies high risk of arrhythmic events, including rapidly conducting preexcited AF (Class IIa).

Cohort data: 9% malignant AF, 2% VF over ~8 years; ablation reduced arrhythmic events in RCT

Occupation-based indication: Catheter ablation is reasonable when pre-excitation precludes specific employment (e.g., pilots) (Class IIa).

Indications and cautions for ablation

When ablation considered: Catheter ablation may be reasonable when medical therapy is not effective or contraindicated (Class IIb); ablation carries a reported 5%–10% risk of AV block and is generally reserved for highly symptomatic patients.

Cryoablation may be used to reduce AV block risk; avoid attempting ablation for transient iatrogenic junctional tachycardia during AVNRT slow-pathway ablation.

Ongoing management — SVT in Adult Congenital Heart Disease (ACHD)

Recommendations and outcomes for ablation in ACHD patients

Surgical/preoperative ablation for Ebstein anomaly: Preoperative catheter ablation or intraoperative surgical ablation of accessory pathways or atrial tachycardia is reasonable in patients undergoing surgical repair of Ebstein anomaly (Class IIa).

Evidence: high prevalence of SVT in Ebstein anomaly; integrated arrhythmia intervention during repair shown safe and effective.

Recurrent symptomatic SVT in ACHD: Catheter ablation is reasonable for treatment of recurrent symptomatic SVT in ACHD patients (Class IIa); acute success rates vary by substrate (65%–100%) and are generally lower in complex anatomy; refer to experienced centers.

Children and complex anatomy may have lower success; acute success for AVNRT >80%; accessory pathways 75%–89% in Ebstein anomaly.

Ongoing management — Pregnant patients with SVT

Acute and ongoing management options in pregnancy with fetal radiation considerations

When to ablate in pregnancy: Catheter ablation may be reasonable in pregnant patients with highly symptomatic, recurrent, drug-refractory SVT with efforts to minimize radiation exposure (Class IIb); avoid first trimester when possible and use radiation-reduction technologies.

Document attempts to minimize radiation exposure and rationale for proceeding.

Acute treatment options: Vagal maneuvers and adenosine are recommended for acute treatment of regular SVT; synchronized cardioversion recommended for hemodynamically unstable SVT; IV diltiazem/verapamil and IV beta blockers reasonable in stable cases.

Class I recommendations for vagal maneuvers and adenosine; various evidence levels.

Ongoing medical therapy in pregnancy: Oral beta blockers, diltiazem, or verapamil useful for ongoing management when no pre-excitation; flecainide or propafenone reasonable without structural heart disease; sotalol, dofetilide, amiodarone, or digoxin may be reasonable or considered in selected refractory cases.

Retrospective and limited data with notable complications reported (e.g., high pericarditis rate in one hybrid epicardial series).

AF ablation adjunctive SVT/AFL ablation

Covered when supported by guideline recommendation

Ablation of additional clinically significant SVT during AF ablation: When a patient is undergoing AF ablation, ablation of previously documented or inducible sustained SVT or atrial flutter can be performed to reduce likelihood of recurrent arrhythmias.

Class IIa recommendation; Level of Evidence B-NR.

This policy does not address transcatheter ablation for premature ventricular contractions (PVCs) or other ventricular arrhythmias in individuals under 18 years of age; separate guidance applies for pediatric ventricular ablation and atrial fibrillation–specific procedures. Refer to the applicable pediatric or AF coverage policies for coverage determinations specific to those populations.

Iatrogenic junctional tachycardia that appears transiently during or after slow-pathway ablation for AVNRT should not routinely be targeted with additional ablation. Such junctional tachycardia is generally benign, can be distinguished from AVNRT during EP study, and attempts to ablate it are unnecessary and may increase the risk of atrioventricular block.

Transcatheter ablation for premature atrial contractions (PACs) and cardioneuroablation (CNA) for reflex syncope or functional bradyarrhythmias is not supported by sufficient peer-reviewed evidence and therefore is not routinely covered. Professional society statements and the literature identify a lack of well-designed randomized or sham-controlled trials for these indications.

No National Coverage Determination (NCD) or Local Coverage Determination (LCD) for these services was identified in this document. Reviewers and providers should check current Medicare NCDs and LCDs, as applicable, to confirm any Medicare-specific coverage rules that could affect determination.

Cardio-neuroablation (CNA) and thoracoscopic epicardial ablation (including hybrid convergent approaches) are considered experimental, investigational, or unproven for all indications in this policy. Available studies are limited, often retrospective, and randomized controlled data are lacking, so these techniques are not supported for routine coverage.

When junctional tachycardia develops transiently as an iatrogenic effect of slow-pathway ablation for AVNRT, attempting to ablate that transient junctional rhythm is not indicated and may cause harm. Because of the risk of atrioventricular block associated with ablation in this region, recognition and conservative management are recommended rather than further ablation attempts.

Cardio-neuroablation (CNA) currently has insufficient evidence to establish safety and efficacy for any indication; consequently, CNA is considered not medically necessary for routine coverage outside of well‑designed clinical trials or other approved research protocols.

Ablation for Accessory Pathway in Asymptomatic Patients

Background and Scope

Supraventricular tachycardia (SVT) arises from tissue at or above the His bundle and includes atrioventricular nodal reentrant tachycardia (AVNRT), atrioventricular reentrant tachycardia (AVRT), focal atrial tachycardias, and macroreentrant atrial tachycardias such as typical cavotricuspid isthmus (CTI)–dependent atrial flutter. Diagnosis is based on ECG and ambulatory monitoring; invasive electrophysiology (EP) study can define the arrhythmia mechanism and allow definitive treatment with catheter ablation. Radiofrequency energy is the most commonly used modality, with cryoablation as an alternative when minimizing AV node injury is important.
