Coverage Criteria for Outpatient BNP / NT-proBNP Testing

inv-01: Medically Necessary Indications

Outpatient testing of plasma BNP or NT-proBNP is considered medically necessary when ANY of the following indications are met:

Heart failure diagnosis and monitoring: Measurement of BNP or NT-proBNP is useful for diagnosis, risk stratification, and monitoring of heart failure and to assist decisions about imaging and referral to advanced therapies.see thresholds module

ACC guidance: biomarkers useful for monitoring; lowering NT-proBNP to < 1,000 pg/mL associated with reverse remodeling in some studies but not established as a treatment target.

Aortic stenosis evaluation: In apparently asymptomatic patients with severe aortic stenosis (Stage C1) and low surgical risk, elevated BNP (> 3 times normal) can support consideration for aortic valve replacement (AVR).>3x normal

Syncope-related testing: Utility of BNP testing is uncertain for suspected cardiac causes of syncope; evidence is insufficient to show influence on outcomes or clinical decision-making.

Outpatient measurement of plasma BNP or NT-proBNP is covered only for the specific clinical indications listed in this policy (for example: evaluation of dyspnea to distinguish heart failure from primary lung disease; timing considerations in asymptomatic severe aortic stenosis; risk stratification and monitoring in chronic heart failure; select perioperative risk assessment; transplant evaluation and waitlist surveillance; suspected cardiac amyloidosis; oncology-related monitoring; and select pediatric transplant surveillance). Testing ordered for any other outpatient indication, including as part of a cardiovascular disease risk panel or profile, is not covered or reimbursable.

Use of BNP or NT-proBNP to direct or target therapy to specific peptide thresholds is considered experimental and not supported by current evidence. Similarly, inclusion of BNP/NT-proBNP in broad screening or proprietary panel tests (including AI-driven panels) is considered experimental and may be denied. Evidence cited in guideline updates and randomized trials (for example, GUIDE-IT) does not demonstrate improved long-term outcomes from biomarker-guided treatment strategies.

Any outpatient indication not explicitly listed in the medically necessary section of this policy is considered not covered or not reimbursable. Proprietary/panel algorithm tests specifically listed (codes 0309U and 0310U) are not covered. Claims for BNP/NT-proBNP ordered for indications outside the policy-supported uses will be denied unless plan-specific benefit terms provide otherwise.

Some clinical scenarios remain of uncertain value and are therefore not supported by this policy. Examples include routine use of BNP testing in the evaluation of syncope where the ACC/AHA/HRS guideline states the utility is 'uncertain', and use of BNP/NT-proBNP within unproven AI-driven cardiac panels or similar algorithm-based blood tests — these uses lack sufficient evidence to influence outcomes or decision-making and are considered experimental.

Background on BNP and NT-proBNP

B-type natriuretic peptide (BNP) and its N‑terminal fragment (NT‑proBNP) are cardiac-derived hormones released with myocardial stretch and ventricular distension. Measurement of these peptides is commonly used as an adjunct to clinical assessment in the evaluation, risk stratification, and monitoring of heart failure, and can help distinguish cardiac from primary pulmonary causes of dyspnea.
