Concomitant surgical ablation: For patients with AF undergoing cardiac surgery, concomitant surgical ablation is recommended or can be beneficial to reduce recurrent AF (supported by ACC and STS recommendations).

inv-15: Active esophageal cooling device (ensoETM) coverage stance — Evidence summary and current stance

Evidence summary and current stance

EIU conclusion: Current evidence is limited; ensoETM results in fewer esophageal thermal injuries and possibly fewer atrioesophageal fistula formations compared with temperature monitoring, but evidence is low‑confidence and insufficient to establish effects on arrhythmia recurrence or MACCE; no comparisons with other protection methods have been reported.

Percutaneous transcatheter left atrial appendage (LAA) closure (CPT 33340) is considered not medically necessary when performed for indications outside the criteria stated in this policy. The policy limits coverage to patients with nonvalvular atrial fibrillation who have a CHA2DS2‑VASc score ≥2 and a documented contraindication to long‑term oral anticoagulation due to a nonreversible cause. Percutaneous LAA closure for any other indication (for example, hypertrophic cardiomyopathy or other uses not meeting the above criteria) is considered not medically necessary.

Surgical closure or exclusion of the LAA performed as a standalone procedure (not performed in conjunction with open cardiac surgery; CPT 33267, 33269) is considered not medically necessary. Closure of a peridevice leak after LAA occlusion (CPT 33999) is considered experimental, investigational, or unproven.

When long‑term oral anticoagulation (OAC) is feasible and acceptable to the patient, percutaneous left atrial appendage occlusion (pLAAO, CPT 33340) is not preferred as a primary stroke‑prevention strategy because the evidence base for OAC is more extensive. Guideline panels (SCAI/HRS, ACC) suggest that for patients who are candidates for either therapy, OAC and LAAO remain treatment options; pLAAO is generally reserved for individuals with CHA2DS2‑VASc ≥2 who have a contraindication to long‑term OAC or for patients at high bleeding risk where patient preference, procedural risk, and careful consideration of alternatives justify LAAO. Coverage determinations should therefore confirm that anticoagulation is not a reasonable option before approving pLAAO.

Epicardial LAA exclusion systems (for example, the AtriClip family and other devices with 510(k) clearance) have regulatory clearance for use under direct visualization in conjunction with open cardiac surgery, but these devices are NOT approved for standalone use for stroke prevention outside of cardiac surgery. Standalone thoracoscopic or epicardial clip approaches have limited prospective and retrospective data suggesting feasibility, but randomized controlled evidence and long‑term outcome data are lacking; as such, these epicardial systems are not considered alternative approved standalone stroke‑prevention therapies.

When concomitant cardiac surgery is not being performed, the role of stand‑alone surgical Maze or modified Maze procedures (including thoracoscopic or endoscopic approaches and hybrid convergent strategies) remains uncertain because evidence is heterogeneous and patient selection criteria are not well defined. Although some high‑volume centers report favorable mid‑term outcomes after stand‑alone surgical Maze procedures, the literature is limited by varying lesion sets, nonstandardized outcome measures, selection bias, and lack of randomized long‑term data; therefore routine coverage for stand‑alone surgical Maze procedures outside of guideline‑supported contexts is limited and may require strong supporting documentation.

In prior analyses of hybrid convergent procedures, prior median sternotomy was an exclusion criterion for patient selection. Published retrospective series that informed the hybrid procedure experience excluded patients with previous sternotomy, reflecting selection in the evidence base and informing the policy interpretation of hybrid approach applicability.

The revision summary indicates that several non‑implemented codes and policy statements were removed in the latest updates. Providers should note that the policy history documents removal of non‑implemented codes and statements (refer to the Revision Details) and ensure billing uses currently implemented, covered codes to avoid claim denials.

The policy specifies several exceptions and not‑medically‑necessary positions: catheter ablation for asymptomatic AF is considered not medically necessary except where explicit exceptions apply; Vein of Marshall (VOM) ethanol infusion (reported with CPT 93799) is currently regarded as not medically necessary or investigational in many contexts given limited and evolving evidence; and standalone surgical LAA exclusion/excision (CPT 33267, 33269) is considered not medically necessary when not performed with open cardiac surgery. These positions reflect current evidence limitations and guideline contexts.

Vein of Marshall (VOM) ethanol infusion is an adjunctive technique evaluated primarily in persistent AF. Although randomized data (eg, VENUS) suggest higher freedom from AF at 6 and 12 months when VOM ethanol infusion is added to ablation in some trials, the overall evidence base is limited by sample size, population diversity, and follow‑up duration. Consequently, VOM ethanol infusion is treated as an evolving/conditional intervention and, in many cases, is considered investigational or not medically necessary pending further larger, long‑term studies to establish safety and durable efficacy.

Stand‑alone thoracoscopic LAA appendectomy and thoracoscopic epicardial clip approaches have demonstrated feasibility and short‑term safety in small prospective and retrospective studies, but the policy notes that the efficacy and long‑term safety have not been established. Randomized controlled trial data evaluating stroke reduction and long‑term outcomes are absent, so stand‑alone thoracoscopic LAA appendectomy is not supported as an established therapy for stroke prevention at this time.

Epicardial ablation that targets atypical lesion sets remains exploratory. The Society of Thoracic Surgeons (STS) and other reviews note promising results in selected centers but emphasize that evidence is heterogeneous and that atypical epicardial lesion strategies lack robust randomized data; therefore such approaches may be considered experimental or not medically necessary until further supportive evidence is available.

The policy considers routine use of the ensoETM active esophageal cooling device (HCPCS C1889) to be experimental/insufficient evidence (EIU). Cohort and registry analyses have suggested fewer esophageal thermal injuries and atrioesophageal fistula events with ensoETM versus temperature monitoring, but large, well‑designed randomized trials demonstrating an impact on major clinical outcomes (including arrhythmia recurrence or major adverse cardiovascular events) are lacking. For these reasons the device remains EIU and may require prior authorization or additional documentation if used.
