Minimum Noninvasive Monitoring Before ILR

Before implanting an ILR/ICM for adults with recurrent or unexplained syncope, a minimum of 14 days of noninvasive ambulatory cardiac monitoring must be performed and be inconclusive or non-diagnostic.

Acceptable noninvasive monitoring includes external patch monitors or external event monitors.
Document the type and duration (≥14 days) of noninvasive monitoring and that it was inconclusive.

Select monitoring modality based on symptom frequency and clinical context, escalating from short-term Holter to extended external patch/event monitoring and then to implantable monitors when indicated.

Daily or near-daily symptoms: consider 24–48 hour Holter monitoring.
Infrequent but recurring symptoms within weeks: consider extended external patch (up to 14–30 days) or event monitors.
Infrequent or unpredictable symptoms (e.g., >30-day intervals) or when external monitoring is nondiagnostic: consider ICM.

Prior Authorization

Noninvasive Monitoring Before Implantable

When symptoms are frequent (recurring within 2–6 weeks), external patch or external loop/event monitoring is typically used prior to considering implantable monitors.

External event monitors or external loop recorders are preferred when symptoms are likely to recur within 2–6 weeks.
Reserved use of implantable monitors for symptoms that are infrequent or when prior external monitoring is nondiagnostic.

Denial Risk

ICM Placement Prerequisites

ICM placement should follow a nondiagnostic initial noninvasive cardiac rhythm monitoring evaluation. Failure to meet these prerequisites or to provide required supporting documentation may result in denial of coverage.

Prerequisite: comprehensive initial evaluation including history, physical exam, 12-lead ECG and ≥14 days of noninvasive monitoring when indicated.
Document why implantable monitoring is necessary and that less invasive monitoring was inconclusive.
High-risk syncopal patients have specific criteria; ensure documentation addresses risk stratification (e.g., CHA2DS2VASc when applicable).
