Weight Loss - Glucagon-Like Peptide-1 Agonists Prior Authorization Policy
Defines Cigna's prior authorization coverage policy for liraglutide (Saxenda), semaglutide (Wegovy), and tirzepatide (Zepbound) for weight management and related indications, affecting providers submitting authorization requests under Cigna-administered health benefit plans.
Zepbound was added to the policy and new criteria were created for it.
Baseline BMI/weight criteria were modified to clarify 'baseline' refers to measurements prior to any GLP-1 or GLP-1/GIP agonist (e.g., Saxenda, Wegovy, Zepbound) rather than prior to initiation of a specific product.
Dosing-related continuation requirements (tolerating maintenance dose or titration windows) were removed and approval duration for continuing therapy standardized to 1 year for Wegovy and Zepbound (and adjusted for Saxenda).
Wegovy indication expanded to include major adverse cardiovascular event (MACE) risk reduction in patients with established cardiovascular disease who are overweight or obese, with clarification re: prior stroke excluding TIA.
Obstructive sleep apnea (moderate to severe) in a patient with obesity was added as an FDA-approved indication for Zepbound.
Concomitant use with other GLP-1 or GLP-1/GIP agonists or other FDA-approved weight-loss medications is not recommended/clarified in conditions not covered.
Wegovy added for metabolic dysfunction-associated steatohepatitis (MASH)/NASH as a condition of approval with documentation and specialist-prescribing requirement.
Liraglutide (generic to Saxenda) was added to the policy and targeted for prior authorization alongside Saxenda, Wegovy, and Zepbound.
