Berinert or Cinryze - Initial Therapy

Intravenous (IV) C1 Esterase Inhibitors (Berinert or Cinryze) are medically necessary when the following apply depending on indication.

Product selection: Berinert or Cinryze. Individual meets ONE of the following:

Prophylaxis (HAE): Diagnosis confirmed by ONE of: (i) confirmed pathogenic variant in SERPING1, F12, ANGPT1, PLG or KNG1 OR (ii) one C4 level below the laboratory lower limit of normal AND ONE of: low functional C1-INH (<50% of normal) OR low antigenic C1-INH (<50% of normal); Berinert or Cinryze will not be concomitantly administered with other FDA‑approved prophylactic HAE treatments (e.g., Haegarda, Takhzyro, Orladeyo); medication is prescribed by, or in consultation with, an allergist/immunologist.

Dosing: age ≥12 up to 2,500 units (≤100 IU/kg) IV no more frequently than twice weekly (doses ≥3 days apart); <12 up to 1,000 units IV no more frequently than twice weekly (doses ≥3 days apart).

Acute treatment (HAE): Diagnosis confirmed by ONE of: (i) confirmed pathogenic variant in SERPING1, F12, ANGPT1, PLG or KNG1 OR (ii) one C4 level below the laboratory lower limit of normal AND ONE of: low functional C1-INH (<50% of normal) OR low antigenic C1-INH (<50% of normal); Berinert or Cinryze will not be concomitantly administered with other FDA‑approved acute HAE treatments (e.g., Firazyr, icatibant, Kalbitor, Ruconest, Sajazir); medication is prescribed by, or in consultation with, an allergist/immunologist; Non‑Covered Product Criteria is met.

Dosing: up to 20 IU/kg IV no more frequently than once daily.

Ruconest - Initial Therapy

Ruconest is medically necessary for acute HAE attacks when ALL of the following are met:

Acute treatment (Ruconest): Diagnosis confirmed by ONE of: (i) confirmed pathogenic variant in SERPING1, F12, ANGPT1, PLG or KNG1 OR (ii) one C4 level below the laboratory lower limit of normal AND ONE of: low functional C1-INH (<50% of normal) OR low antigenic C1-INH (<50% of normal); Ruconest will not be concomitantly administered with other FDA‑approved prophylactic or acute HAE treatments (for example, Berinert, Cinryze, Firazyr, icatibant, Kalbitor, or Sajazir); medication is prescribed by, or in consultation with, an allergist/immunologist; Non‑Covered Product Criteria is met.

Dosing: up to a maximum of 4,200 units (not exceeding 50 IU/kg) IV no more frequently than twice daily.

Continuation Therapy / Reauthorization

Continuation of therapy is considered medically necessary when evidence of benefit since initiation and prescriber involvement are documented.

Berinert/Cinryze continuation (Prophylaxis): Prior medical necessity criteria were met before initiating Berinert or Cinryze AND there is documentation of beneficial response since starting prophylactic therapy (for example, decreased HAE attack frequency, decreased attack severity, or decreased duration of attacks) AND medication continues to be prescribed by, or in consultation with, an allergist/immunologist.

Berinert/Cinryze continuation (Acute): Prior medical necessity criteria were met before initiating Berinert or Cinryze AND there is documentation of beneficial response since initiating therapy (for example, decreased duration of HAE attacks, quick onset of symptom relief, complete resolution of symptoms, or decreased attack frequency/severity) AND medication continues to be prescribed by, or in consultation with, an allergist/immunologist.

Ruconest continuation: Prior medical necessity criteria were met before initiating Ruconest AND there is documentation of beneficial response since initiating Ruconest therapy (for example, decreased duration of HAE attacks, quick onset of symptom relief, complete resolution of symptoms, or decreased attack frequency/severity) AND medication continues to be prescribed by, or in consultation with, an allergist/immunologist.

Ruconest (recombinant C1‑esterase inhibitor) for prophylaxis of hereditary angioedema (HAE) is considered experimental, investigational, or unproven and not covered. The evidence to support Ruconest for prevention of HAE attacks is currently insufficient despite a small Phase II trial showing benefit; Ruconest is not FDA‑approved for HAE prophylaxis. Episodes of angioedema with normal C1‑inhibitor levels/function that are not associated with a documented pathogenic variant in F12, ANGPT1, PLG, or KNG1 are also excluded from coverage.

Any use of C1‑esterase inhibitor products that is not specifically supported by the policy’s medical necessity criteria is considered experimental, investigational, or unproven and therefore not covered. This includes, but is not limited to, Ruconest for HAE prophylaxis. The Conditions Not Covered list should be consulted for other specific exclusions.

Prior Authorization

Prior Authorization Required

Prior authorization (PA) is required. Submit diagnostic documentation and prescriber specialty with the PA request. Include dosing and coverage rules applicable to the requested product to support medical necessity determinations.

PA must include diagnostic confirmation (see Diagnostic and Prescriber Documentation callout).
Include prescriber specialty or documentation of consultation with an allergist/immunologist.
Provide proposed dosing regimen and intended frequency/site of care.
Operational: review processing may deny or pend if documentation of prior therapies or age restrictions from Non-Covered Product Triggers is missing.

Documentation Required

Diagnostic and Prescriber Documentation

Required diagnostic confirmation and prescriber documentation must be provided with the PA. Acceptable diagnostic evidence includes either a documented pathogenic variant in a relevant gene OR laboratory evidence of complement abnormalities. Prescriber requirement: medication must be prescribed by, or in consultation with, an allergist/immunologist.

Diagnostic confirmation (ONE of the following): confirmed pathogenic variant in SERPING1, F12, ANGPT1, PLG, or KNG1; OR one C4 level below the laboratory lower limit of normal AND one of: low functional C1‑INH (<50% of normal) at baseline OR low C1‑INH antigenic level (<50% of normal) at baseline.
Prescriber specialty: prescribed by or in consultation with an allergist/immunologist.
If PA is for prophylaxis vs acute treatment, indicate which indication and include dosing plan (age-based dosing limits apply).

Denial Risk

Non-Covered Product Triggers

Non-coverage triggers that can cause a product to be non-covered or require additional justification are listed below. Confirm age, current therapy status, and prior use of icatibant where required.

Age restrictions: requests for Berinert, Cinryze, or Ruconest for individuals < 18 years may be non-covered per plan criteria unless specific justification is provided.
Current therapy: if the individual is currently receiving the requested product (Berinert, Cinryze, or Ruconest), coverage may be denied for duplicate therapy or continuation rules — document ongoing use if continuation is intended.
Icatibant (Sajazir) requirement: documentation of failure, contraindication, or intolerance to icatibant may be required prior to coverage for Berinert, Cinryze, or Ruconest where the plan specifies.
Operational note: when Non-Covered Product Triggers apply, include clinical rationale and supporting records to avoid denial (e.g., prior therapy notes, adverse reaction documentation, age-based justification).

Step Therapy

Step Requirement: Icatibant

Some plans require trial, failure, contraindication, or intolerance to icatibant (Sajazir) before covering IV C1 esterase inhibitors for certain indications. Verify plan-specific step requirements during PA submission.

If plan requires step therapy with icatibant, submit documentation of trial and clinical outcome (failure, contraindication, or intolerance) with the PA.
If icatibant is contraindicated or not appropriate, include supporting clinical rationale and relevant records.
