Initial Therapy

Covered when ALL of the following are met

HAE - Treatment of Acute Attacks: Individual meets ALL of the following criteria:

A. Diagnosis confirmation: One of: (i) Confirmed pathogenic variant in SERPING1, F12, ANGPT1, PLG or KNG1 OR (ii) One C4 level below the lower limit of normal as defined by the performing laboratory AND one of: (a) low functional C1-INH (<50% of normal) at baseline OR (b) low C1-INH antigenic level (<50% of normal) at baseline.<50% when referenced

Laboratory reference values must document percentages as specified.

B. Concomitant therapy: Icatibant will not be concomitantly administered with other FDA-approved treatments for acute HAE attacks (for example, Berinert, Cinryze, Kalbitor, or Ruconest).

C. Prescriber: Medication is prescribed by, or in consultation with, an allergist/immunologist.

D. Non-covered product criteria: Plan-specific non-covered product criteria are met as applicable (refer to non-covered product tables).

Reauthorization

Continuation is covered when ALL of the following are met

1. Prior criteria met: The above medical necessity criteria were met prior to the start of icatibant therapy.

2. Clinical benefit: Documentation of beneficial response since initiating icatibant (for example, decreased duration of HAE attacks, quick onset of symptom relief, complete resolution of symptoms, or decreased attack frequency or severity).

3. Prescriber: Medication continues to be prescribed by, or in consultation with, an allergist/immunologist or a physician who specializes in the treatment of HAE or related disorders.

Icatibant is not indicated for prophylaxis of hereditary angioedema (HAE) attacks and, for that reason, use of icatibant for prophylactic therapy is considered experimental/investigational and not covered.

Use of icatibant for episodes of angioedema when C1‑inhibitor levels and function are normal is considered experimental/investigational unless the angioedema is associated with a documented pathogenic variant in one of the specified genes (F12, ANGPT1, PLG, or KNG1).

Certain plan formulations may list Firazyr (icatibant) as a non‑covered product. For employer group and individual/family plans where this applies, coverage exceptions require documentation that the member trialed the bioequivalent product (Sajazir or icatibant) and cannot take it because of a clinically significant allergy or serious adverse reaction to an inactive ingredient (formulation difference).

Required Clinical Documentation

Requests must include clinical documentation confirming diagnosis and evidence of meeting medical necessity criteria. Documentation should also support lack of contraindicated concomitant therapy and specialist involvement.

Background

Icatibant is a synthetic decapeptide indicated for the treatment of acute hereditary angioedema (HAE) attacks in adults (≥18 years). Clinical guidance (US HAE Association 2020; WAO/EAACI 2021) supports on‑demand treatment of HAE attacks, and lists icatibant among first‑line therapies for acute attacks alongside plasma‑derived C1‑INH and ecallantide. Appropriate diagnostic testing for HAE includes serum C4 and C1‑INH antigenic and functional levels; low C4 with low functional or antigenic C1‑INH supports a diagnosis of HAE types I/II. Per policy, icatibant is covered when medical necessity criteria are met and prior authorization requirements are satisfied.
