PTNS (percutaneous posterior tibial nerve stimulation) is covered with medical necessity criteria: diagnosis of OAB, failure of conservative medical management (unless not desired or contraindicated), and provision of the standard regimen of 30-minute weekly sessions for 12 weeks. Once-a-month maintenance PTNS is medically necessary for patients who experience significant improvement after the initial 12 treatments. The policy states there is insufficient evidence to support use of PTNS beyond 12 months when there is no improvement, and that implantable tibial nerve stimulation devices are not supported by the published peer-reviewed literature.

PTNS is medically necessary when ALL of the following are met:

Diagnosis of OAB: Diagnosis of overactive bladder (OAB).
Failure of conservative management: Failure of conservative medical management unless conservative management is not desired or is medically contraindicated.
Conservative management examples include behavioral therapies such as bladder training or pelvic floor muscle training, and pharmacotherapy with oral medications used with the intent to treat OAB.
Standard treatment regimen: Service is provided in accordance with the standard treatment regimen of 30-minute weekly sessions for 12 weeks.30-minute weekly sessions for 12 weeks

Use appropriate CPT for percutaneous PTNS

Report CPT 64566 for percutaneous posterior tibial neurostimulation (percutaneous needle electrode, single treatment, includes programming). Do not report implantable neurostimulation CPT codes for percutaneous PTNS; those implantable/system programming codes are not supportive for percutaneous PTNS.

Background & Evidence

Background: PTNS is a minimally invasive percutaneous neuromodulation therapy for OAB involving insertion of a fine needle electrode near the posterior tibial nerve (approximately 5 cm cephalad to the medial malleolus and 2 cm posterior to the tibia) with a surface foot electrode and connection to a low-voltage stimulator. Stimulation is typically delivered at 0.5–9 mA at 20 Hz for 30 minutes. The usual initial regimen consists of 12 weekly 30-minute sessions, with some responders showing improvement after 6–8 sessions; maintenance sessions may be required to sustain response and are commonly once a month for responders. Several implantable tibial nerve neuromodulation systems (including leadless miniature implants) are under investigation, but the evidence on their benefits and efficacy is currently limited / insufficient to support their use for urinary voiding dysfunction.
