Initial Therapy — Covered when ALL of the following are met

Initial SIJ injection criteria: A. One injection per visit; B. Moderate to severe low back pain primarily over the anatomical SIJ region for at least 3 months; C. Low back pain below L5 without radiculopathy; D. Positive response to at least three SIJ provocation tests (e.g., distraction, compression, thigh thrust, Gaenslen's, FABER/Patrick's, sacral thrust); E. Failure to respond to a minimum of 4 weeks of conservative therapy (comprehensive exercise program, local icing, mobilization/manipulation, NSAIDs); F. Clinical findings and/or imaging do not indicate another diagnosed cause of lumbosacral pain (e.g., central spinal stenosis, foraminal stenosis, disc herniation with concordant radiculopathy, infection, tumor, fracture, pseudoarthrosis, or pain related to spinal instrumentation); G. Injections performed under CT or fluoroscopy image guidance with contrast (ultrasound only when documented contrast allergy or pregnancy); H. SIJ injections are not performed concurrently with other lumbosacral musculoskeletal injections and documentation shows direct causal benefit from the SIJ injection.

Extended continuation criteria

Sacroiliac nerve blocks and radiofrequency neurotomy (conventional, cooled, and pulsed) are classified as not medically necessary for the treatment of sacroiliac joint (SIJ) pain because effectiveness has not been established. High-quality evidence is lacking for conventional and pulsed radiofrequency neurotomy, and cooled radiofrequency neurotomy requires additional well-designed studies to evaluate effectiveness.

Sacroiliac nerve blocks and radiofrequency neurotomy are considered not medically necessary for treatment of SIJ pain because the effectiveness of these procedures has not been established. The literature reports mixed results for radiofrequency neurotomy, including at least one randomized study finding no difference between conventional RFA and sham treatment.

For cooled radiofrequency neurotomy specifically, available systematic reviews indicate short-term improvements in pain but the body of evidence is insufficient; additional well-designed studies are needed before cooled RFA can be considered established therapy.

Background

Sacroiliac joint (SIJ) pain is an identifiable cause of chronic low back pain in a substantial subset of patients and SIJ injections are used both diagnostically and therapeutically. When indicated, SIJ injections performed under image guidance can provide immediate pain relief and may help avoid more invasive procedures when conservative measures fail. Risks associated with SIJ injections include nerve injury, hematoma, infection and myotoxicity. For long‑term management, repeated procedures should be coordinated with the primary care provider for monitoring of prolonged steroid exposure and multidisciplinary care.
