Coverage Criteria for Caudal and Interlaminar ESIs

inv-01: Acute ESI — Covered when ALL of the following are met

Covered when ALL of the following are met:

Acute management (single caudal or interlaminar ESI): 1) Severe radicular pain that interferes substantially with activities of daily living (ADLs); 2) Pain lasting less than three months; 3) Severe pain persists after treatment with NSAID and/or opiate therapy for three days unless contraindicated or not tolerated; 4) The member cannot tolerate chiropractic or physical therapy and the injection is intended as a bridge to therapy; 5) Only one procedure is performed per visit and imaging guidance (fluoroscopy or CT) is required except in rare instances with documented justification.

inv-02: Initial ESI for chronic pain — Covered when ALL of the following are met

Covered when ALL of the following are met:

Initial ESI for chronic pain: 1) Request is for one caudal or interlaminar ESI at one level in the cervical, thoracic, or lumbar region; 2) Persistent radicular pain caused by spinal stenosis, disc herniation, or degenerative vertebral changes confirmed by physical exam and imaging; 3) Pain interferes with ADLs and has lasted at least three months; 4) Failure of conservative therapy including: a) four weeks of chiropractic, physical therapy, or prescribed home exercise program; b) four weeks of activity modification; c) NSAID for three weeks or documentation that NSAID is contraindicated or not tolerated.

inv-03: Second ESI — Covered when ALL of the following are met

Covered when ALL of the following are met:

Second ESI after inadequate response: 1) Request is for an ESI at one level in the cervical, thoracic, or lumbar region; 2) At least two weeks have passed since the first ESI; 3) The first ESI did not lead to sufficient improvement.

inv-04: Subsequent ESI after documented benefit — Covered when ALL of the following are met

Covered when ALL of the following are met:

Subsequent ESI for recurrence after prior benefit: 1) Initial injection(s) produced ≥ 50% pain relief and functional improvement for at least two months; 2) At least two months have passed since the last ESI; 3) Fewer than four injections have been administered within the prior 12 months; 4) Less than 12 months have elapsed since the initial injection at the level requested; 5) Request is for an ESI at one level in the cervical, thoracic, or lumbar region.≥ 50% pain relief

Caudal or interlaminar epidural steroid injections (ESIs) for indications or anatomic locations that are not specified in the coverage criteria are considered not medically necessary because effectiveness has not been established. This exclusion applies to requests that fall outside the defined situations for acute management, initial chronic pain management, second ESI after inadequate response, or subsequent ESI following documented prior benefit.

Coverage determinations made under this clinical policy are subject to the terms, conditions, exclusions, and limitations of the member's coverage documents (for example, evidence of coverage, certificate of coverage, policy, or contract of insurance). Where state Medicaid provisions conflict with this clinical policy, the state provisions take precedence. The Health Plan retains the right to change, amend, or withdraw this clinical policy and may apply Health Plan–level administrative policies and procedures when making coverage decisions.

A third or subsequent caudal or interlaminar ESI for chronic pain that did not improve from the first two ESIs is considered not medically necessary. Requests for additional injections after two unsuccessful ESIs will be denied absent documentation of prior benefit, since effectiveness for continued interventions in this context has not been established.

Required Conservative Treatment Before ESI

inv-22: Documented trial and failure or intolerance — PT/home exercise 4 weeks, activity modification 4 weeks, NSAID 3 weeks

Documentation and Clinical Requirements

Imaging guidance (fluoroscopy or CT) is the default requirement for caudal or interlaminar ESIs. Rare exceptions to imaging guidance may be considered only with clear, documented justification. Conservative therapy is required prior to authorization: members should have tried and failed or been unable to tolerate conservative measures (e.g., NSAIDs, opioids when appropriate, and chiropractic or physical therapy) unless contraindicated. The injection may be authorized as a bridge to therapy when the member cannot tolerate conservative treatments. Providers must document clinical indications (e.g., severe radicular pain substantially interfering with activities of daily living), prior conservative therapy, and rationale for any deviation from imaging guidance or frequency limits.
