Coverage Criteria for Sacroiliac Joint Fusion

Open SIJ fusion - Indications

Open SIJ fusion is medically necessary for the following specific indications:

Open SIJ fusion indications: Stabilization of a traumatic, severe disruption, or fracture of the pelvic ring; as an adjunct to sacrectomy for treatment of sacral tumors; as an adjunct to treatment of sacroiliac joint infection or sepsis (e.g., osteomyelitis, pyogenic sacroiliitis); during multi-segment spinal constructs extending to the ilium for correction of spinal deformity (e.g., scoliosis or kyphosis).

Minimally invasive SIJ fusion - Initial criteria

Covered when ALL of the following are met:

Conservative therapy: Failure of at least six consecutive months of conservative treatment unless contraindicated, including medication optimization and activity modification; at least four to six weeks of active therapeutic exercise targeted at the lumbar spine, pelvis, SIJ and hip (including a home exercise program) or documented inability to tolerate such exercise.6 months

From criteria II (minimally invasive).

Pain characteristics: Non-radiating, typically unilateral pain caudal to the lumbar spine localized over the posterior SIJ that interferes with activities of daily living; localized posterior SIJ tenderness (sacral sulcus) in the absence of tenderness of similar severity elsewhere; absence of generalized pain disorders (e.g., fibromyalgia).

Pain localization and exclusion of generalized pain disorders.

Provocative tests: Positive response to at least three provocative SIJ tests (e.g., thigh thrust, compression, sacral thrust; distraction, Gaenslen's, Patrick's/FABER test).>=3 tests

Imaging: Recent (within six months) diagnostic imaging including plain radiographs and CT or MRI of the SIJ excluding destructive lesions (tumor, infection), fracture, traumatic SIJ instability, or inflammatory arthropathy; plain radiographs of the pelvis to exclude concomitant hip pathology; CT or MRI of the lumbar spine excluding neural compression or other degenerative conditions causing low back or buttock pain.within 6 months

It is the policy of health plans affiliated with Centene Corporation that the long-term safety and effectiveness of SIJ fusion procedures, either open or minimally invasive, has not been proven for indications other than those specifically listed in this policy (for example, treatment of mechanical or axial low back pain, radicular syndromes, sacral insufficiency fractures, and pelvic girdle pain). Use of implants that do not transfix the joint (for example, intra‑articular stabilization devices) is not supported by the coverage criteria.

Coverage determinations are made in accordance with the member's specific plan documents. This clinical policy provides medical necessity guidance to assist with coverage decisions, but final coverage is subject to the terms, conditions, exclusions, and limitations of the member’s benefit contract and to applicable state and federal requirements. For Medicaid members, state Medicaid provisions take precedence where they conflict with this policy.

Percutaneous SIJ arthrodesis procedures billed using codes for placement of intra‑articular implants without placement of a transfixation device (for example, CPT 27278) are not supported by the coverage criteria and may be denied.

Background and Context

The sacroiliac joint is a recognized source of chronic low back pain, implicated in roughly 15–30% of patients with chronic back symptoms. SIJ fusion is performed to stabilize the joint and may be accomplished via an open approach or by minimally invasive techniques. Minimally invasive procedures are typically performed using a lateral transarticular approach with transfixing implants placed across the ilium into the sacrum to promote arthrodesis. Randomized trials have demonstrated benefit for some lateral minimally invasive SIJ fusion systems versus nonoperative care, but evidence on long‑term outcomes and head‑to‑head comparisons among devices remains limited.
