Initial Therapy Criteria

I. Initial Approval Criteria

Approval of the drug does not translate to an approval of the corneal cross-linking procedure. Photrexa and Photrexa Viscous are medically necessary when ALL of the following are met:

ALL of the following

Diagnosis of one of the following
- Progressive keratoconus
- Corneal ectasia following refractive surgery
Prescribed by or in consultation with an ophthalmologist
Age ≥ 14 years≥ 14 years
Patient must be at least 14 years old
Dose does not exceed one kit per eye≤ one kit per eye
Maximum one kit per eye

I.B. Other diagnoses/indications

If the request is for other diagnoses/indications (must meet ONE of the following):

If this drug has recently (within the last 6 months) undergone a label change that is not yet reflected in this policy, follow the relevant no-coverage or non-formulary policy for the line of business
Refer to CP.CPA.190 (commercial), HIM.PA.33 or HIM.PA.103 (marketplace/ICHRA), CP.PMN.255 or CP.PMN.16 (Medicaid) depending on formulary status
If requested use is not listed under section III and criterion 1 does not apply, refer to the off-label use policy for the relevant line of business

Continuation Therapy Criteria

II. Continued Therapy

Approval of the drug does not translate to an approval of the corneal cross-linking procedure. Continued therapy for Progressive Keratoconus and Corneal Ectasia is medically necessary when ALL of the following are met:

ALL of the following

Member meets one of the following
- Currently receiving medication via Centene benefit or member has previously met initial approval criteria
- Member is currently receiving medication and is enrolled in a state and product with continuity of care regulations
  Refer to state specific addendums CC.PHARM.03A and CC.PHARM.03B
At least 6 months have passed since member's last collagen cross-linking procedure≥ 6 months
Member is responding positively to therapy as evidenced by a reduction in diopters in the treated eye(s)
If request is for a dose increase, new dose does not exceed one kit per eye≤ one kit per eye
Maximum one kit per eye

II.B. Continued Therapy - Other diagnoses/indications

If the request is for other diagnoses/indications (must meet ONE of the following):

If drug label changed within last 6 months not yet reflected, follow relevant no-coverage or non-formulary policy
