Somatosensory EP (SSEP): Medically necessary when ALL of the following apply: (1) to aid prognosis of acute anoxic encephalopathy within the initial 72 hours of onset (e.g., after cardiac arrest); (2) assessment of a decline in status that may warrant emergent surgery in unconscious spinal cord injury patients who have specific structural somatosensory system damage and are candidates for emergency spinal cord surgery; (3) to aid in the diagnosis of multiple sclerosis; (4) to aid assessment of coma following traumatic, hypoxic-ischemic, and other diffuse brain injuries; (5) assessment of central nervous system deficiency identified on clinical exam when not explained by appropriate imaging studies; (6) management of conditions causing spinocerebellar degeneration (e.g., Friedreich's ataxia) or peripheral nerve degeneration (e.g., diabetic neuropathy); and (7) intraoperative monitoring during surgeries that may affect neural structures.

Brainstem Auditory EP (BAEP): Medically necessary when ALL of the following apply: (1) assessment of brainstem function (e.g., during tumor infiltration of the brainstem and after surgical removal of a lesion); (2) diagnosis and monitoring of demyelinating and degenerative diseases affecting the brainstem (e.g., multiple sclerosis, central pontine myelinolysis, olivopontocerebellar degeneration); (3) diagnosis of lesions in the auditory system (e.g., acoustic neuroma); (4) screening for hearing loss in infants and preverbal children or children with developmental delay or intellectual disability; (5) to aid in evaluation of prognosis in coma within the initial 72 hours of onset (excluding evaluation of brain death); and (6) intraoperative monitoring during surgeries that may affect neural structures.

Visual EP (VEP): Medically necessary when ALL of the following apply: (1) diagnosis and monitoring of optic nerve function and during demyelinating disorders of the optic nerve (e.g., multiple sclerosis, optic neuritis); (2) assessment of suspected disorder of the optic nerve, optic chiasm, or pre-chiasmic radiations (note: VEPs are not useful for post-chiasmic disease); and (3) evaluation of visual loss in patients who are unable to communicate.

Intraoperative EP monitoring: Medically necessary when ALL of the following apply: somatosensory evoked potentials, motor evoked potentials using transcranial electrical stimulation, and brainstem auditory evoked potentials are used during intracranial, orthopedic, spinal, and vascular surgeries to monitor neural function and help prevent intraoperative neural injury.

Not medically necessary / insufficient evidence: Evoked potential testing is considered not medically necessary when used for the following indications: motor evoked potentials from transcranial magnetic stimulation; intraoperative monitoring of visual evoked potentials; motor evoked potentials for non‑operative monitoring; visual evoked potentials for glaucoma or glaucoma suspect, amblyopia, or diabetes; and for evaluation/assessment of any other conditions not specified as covered in this policy.

These include motor evoked potentials from transcranial magnetic stimulation, intraoperative monitoring of visual evoked potentials, and motor evoked potentials for non‑operative monitoring. Visual evoked potentials are specifically considered not medically necessary for assessment related to glaucoma or glaucoma suspect, amblyopia, and diabetes. The policy further states that evoked potential testing is not medically necessary for evaluation or assessment of conditions other than those explicitly specified as covered.

Evoked potential testing performed for the indications listed as not medically necessary in this policy is considered not medically necessary due to insufficient evidence in the published peer‑reviewed literature. Claims for these indications (for example, VEP testing for glaucoma, amblyopia, or diabetes, or MEPs for non‑operative monitoring) may be denied when submitted for payment.

Evoked potentials (EPs) are electrophysiologic recordings of electrical responses within central and peripheral nervous system pathways following specific stimulation. Common modalities include somatosensory evoked potentials (SSEP), brainstem auditory evoked potentials (BAEP/ABR), visual evoked potentials (VEP), and motor evoked potentials (MEP). Clinically, EPs are used to aid diagnosis and monitoring of optic nerve and demyelinating disorders (for example, multiple sclerosis and optic neuritis), to assess brainstem and auditory pathway function, to evaluate prognosis in coma or anoxic brain injury, and to provide intraoperative monitoring during intracranial, spinal, orthopedic, and vascular surgeries to help avoid neural injury.
