Coverage Criteria

Initial Therapy — Covered when ALL of the following are met for migraine prophylaxis.

Initial Migraine Prophylaxis: 1. Diagnosis of episodic or chronic migraine; 2. Member experiences > 4 migraine days per month for at least 3 months; 3. Prescribed by or in consultation with a neurologist, headache, or pain specialist; 4. Age ≥ 18 years; 5. Failure of at least 2 oral migraine preventative therapies, each for 8 weeks and from different therapeutic classes (examples: antiepileptic drugs such as divalproex sodium, sodium valproate, topiramate; beta-blockers such as metoprolol, propranolol, timolol; antidepressants such as amitriptyline, venlafaxine), unless clinically significant adverse effects or contraindications exist; 6. Failure of a 3-month trial of at least ONE injectable CGRP therapy used for migraine prophylaxis (e.g., Aimovig, Ajovy, Emgality, Vyepti), unless clinically significant adverse effects or contraindications exist; 7. Qulipta is not prescribed concurrently with other CGRP inhibitors (e.g., Aimovig, Ajovy, Emgality, Nurtec ODT, Ubrelvy, Vyepti, Zavzpret); 8. Dose does not exceed 60 mg (1 tablet) per day.

Botox concurrent therapy: a. Sufficient evidence provided from at least two high-quality, published studies in reputable peer-reviewed journals or evidence-based clinical practice guidelines that include: (i) adequate representation of the member's clinical characteristics, age, and diagnosis; (ii) adequate representation of the prescribed drug regimen; (iii) clinically meaningful outcomes such as reduction in monthly migraine or headache days; and (iv) appropriate experimental design and methods (see Appendix D for additional information); b. Member has experienced and maintained a positive response to Botox monotherapy evidenced by a ≥ 30% reduction in migraine days per month from baseline after at least 2 quarterly Botox injections (6 months); c. Despite Botox monotherapy, member continues to have ≥ 4 migraine days per month and/or severe migraine headaches causing disability or functional impairment.

Case studies or chart reviews are not considered high-quality evidence.

Continuation Therapy — Continued therapy covered when ALL of the following are met.

Continued Therapy: 1. Member meets one of the following: a) currently receiving medication via Centene benefit or previously met initial approval criteria; or b) currently receiving medication and enrolled in a state/product with continuity of care regulations; 2. Member has experienced and maintained a positive response to therapy as evidenced by a reduction in migraine days per month from baseline; 3. Qulipta is not prescribed concurrently with other CGRP inhibitors (e.g., Aimovig, Ajovy, Emgality, Nurtec ODT, Ubrelvy, Vyepti, Zavzpret). (This requirement does not apply to CA if member was previously approved via Centene benefit and is currently stable on therapy with both oral and injectable CGRP inhibitors); 4. If request is for a dose increase, new dose does not exceed 60 mg (1 tablet) per day.

Approval duration: 12 months.

Non‑FDA approved indications that are not specifically addressed by this policy are not authorized unless the provider supplies sufficient documentation demonstrating efficacy and safety in accordance with the referenced off‑label use policies (for example, CP.CPA.09 for commercial, HIM.PA.154 for marketplace, and CP.PMN.53 for Medicaid) or applicable evidence of coverage documents.

Prescriptions for Qulipta (atogepant) concurrently with certain injectable CGRP therapies are restricted. The policy now explicitly includes Zavzpret in the list of CGRP inhibitors that should not be prescribed concurrently with Qulipta and clarifies that when concurrent use is considered, the member must meet the policy's redirection requirements which include a required 3‑month trial of the injectable CGRP therapy used for migraine prophylaxis prior to redirection.

Qulipta must not be prescribed concurrently with other CGRP inhibitors (examples listed in policy include Aimovig, Ajovy, Emgality, Nurtec ODT, Ubrelvy, Vyepti, and Zavzpret), except where state‑specific exceptions apply (for example, California allows continuation if the member was previously approved via the Centene benefit and is currently stable on both oral and injectable CGRP therapies). In all cases, dosing must not exceed 60 mg/day (one 60 mg tablet).
