inv-01: RVPs (<=5 pathogens) — Medical necessity

Covered when ALL of the following are met:

Core criteria for <=5-pathogen RVPs: 1) The member/enrollee is immunocompetent; 2) The clinical indication includes a presumption of active infection or infection-associated complications (which may include exacerbation of underlying disease) that require identification of a causative organism for appropriate management; 3) The results of testing will impact clinical management in a manner demonstrated in peer-reviewed published literature to improve outcomes; 4) Testing is performed according to the intended use of the test in the intended population for which the test was developed and validated; 5) The panel performed includes at least the minimum pathogens required for clinical decision making for its intended use that can be reasonably detected by the test; 6) Targeted testing is not appropriate (i.e., targeted testing will not provide sufficient information for appropriate clinical management); 7) The registered test demonstrates equivalent or superior analytical and clinical validity to established standard-of-care methods (e.g., culture, pathogen-specific PCR) for the majority of targets included on the panel.

Additional situational criteria: 1) Performed in a healthcare setting that cares for critically ill individuals (e.g., emergency department, inpatient hospital, observation status); OR 2) Member/enrollee is immunocompromised (see section I.A.2 definition); OR 3) Member/enrollee is immunocompetent with severe and established underlying respiratory pathology (e.g., severe asthma, COPD, pulmonary fibrosis, prior lung radiation) or when treatment with antibiotics may be indicated according to established guidelines.

inv-03: Panel-size and outpatient influence criteria

Policy distinguishes medical necessity for small panels (≤5 pathogens) versus larger panels (≥6 pathogens) and includes outpatient conditions where testing must influence plan of care.

Panel-size-specific rules: Separate medical necessity statements exist for panels of 5 or fewer pathogens and for panels of 6 or more pathogens. Multiplex PCR respiratory viral panels (RVPs) detect the RNA or DNA of multiple respiratory viruses from upper respiratory specimens and, depending on the assay, can test for a broad array of viral targets. The document underscores that RVPs are generally highly sensitive and specific and can inform antiviral use, infection control decisions, and reduce unnecessary antibiotic prescribing. It also highlights that immunocompromised patients are at higher risk for severe lower respiratory tract disease and may derive greater clinical benefit from multiplex testing, while for many patients the identification of the specific viral pathogen does not change individual management.

Billing Rule

Coding/place-of-service linkage for authorization

Providers must bill the CPT and Proprietary codes listed in the policy using the correct place-of-service or supporting diagnosis linkage to meet authorization/billing requirements.

Certain CPT codes (see policy code tables) support medical necessity in any place of service without a diagnosis code (e.g., 87631).
Other CPT/proprietary codes require a specific place of service (see Table 3) or an ICD-10 diagnosis from the policy’s diagnosis tables to support medical necessity (see Table 2 description).
Refer to the most current professional coding guidance when submitting claims.

Billing Rule

Code-table billing requirements

Some CPT and proprietary molecular panel codes are medically necessary only when billed with the POS codes or ICD-10 diagnosis codes specified in the policy tables.

Codes that support medical necessity in any POS: e.g., 87631.
Codes that require POS linkage or specific diagnosis codes: see Table 2 (examples include 0115U and other listed U-codes) and Table 3 for required place-of-service values.
Claims billed without the required POS or diagnosis linkage may be denied.

Denial Risk

Denial risk when core criteria not met

Testing for five pathogens or fewer is medically necessary only when core clinical criteria are documented; absence of these criteria risks claim denial.

Medical record must show clinical indication consistent with suspected active infection or infection-associated complications that require identification of a causative organism.
Testing for ≤5 pathogens is medically necessary when results will impact clinical management as supported by peer-reviewed literature and the test is used in its intended population.
Failure to meet these core criteria (e.g., no active symptoms, no expected impact on management) may result in denial.

Denial Risk

Denial risk for larger panels without additional criteria

Panels that detect six or more pathogens require additional situational criteria; billing without meeting these higher-threshold criteria may be denied.

Multiplex panels (≥6 pathogens) are considered medically necessary only when performed in settings that care for critically ill patients (ED, inpatient, observation) and when core criteria are also met.
Alternatively, medical necessity is met if the member is immunocompromised or has severe underlying respiratory pathology or other listed situational criteria.
Use targeted testing when appropriate; routine use of large multiplex panels without these criteria risks denial.

When a CPT/proprietary code requires a supporting ICD-10 diagnosis from the policy tables, omission of an appropriate diagnosis code can result in claim denial.
When a code requires a specific place of service (Table 3), billing from an unsupported POS can lead to denial.
Verify that submitted codes match the policy’s tables and the clinical documentation provided.

Clinical documentation must clearly support the medical necessity of the test; inadequate documentation may lead to claim denial or audit.

Document the specific clinical indication and clinical suspicion for the pathogen being tested.
Record specific reasons for performing the test/panel, including how results will influence patient management.
Include provider type/specialty and place of service in the medical record.
For immunocompromised patients, document the relevant immunosuppressive condition or therapy.

Step Therapy

Targeted vs multiplex testing

Targeted testing is preferred when sufficient clinical suspicion exists; multiplex panels should be avoided when not necessary.

Targeted single- or limited-analyte testing should be used when a specific pathogen is suspected and will inform management.
Multiplex panels detecting many pathogens are discouraged unless criteria for larger panels are met (e.g., critically ill setting, immunocompromised, severe underlying respiratory disease).
There is no stated step therapy, but clinical justification for multiplex testing must be documented.
