Initial Therapy (SLE)

Covered when ALL of the following are met:

SLE initial approval: Member has a diagnosis of systemic lupus erythematosus; prescribed by or in consultation with a rheumatologist; age ≥ 5 years; prescribed in combination with standard therapy for SLE (one or more of: glucocorticoids, antimalarial, or non-biologic immunosuppressant) unless contraindicated; documentation of positive SLE autoantibody (e.g., ANA, anti-dsDNA, anti-Sm, anti-RNP, anti-Ro/SSA, anti-La/SSB); belimumab is not prescribed concurrently with Lupkynis (voclosporin) or another biologic agent; dosing is within specified IV or SC limits as described in the dosing section.

Initial Therapy (Lupus Nephritis)

Covered when ALL of the following are met:

LN initial approval: Diagnosis of lupus nephritis with kidney biopsy confirming class III, IV, or V; prescribed by or in consultation with a nephrologist or rheumatologist; age ≥ 5 years; confirmed diagnosis of SLE; prescribed in combination with standard therapy for SLE (one or more of: glucocorticoids, antimalarial, or non-biologic immunosuppressant, including cyclophosphamide when appropriate) unless contraindicated; belimumab is not prescribed concurrently with Lupkynis (voclosporin) or another biologic agent; dosing meets the LN-specific IV or SC loading and maintenance regimens; loading (LN-specific) doses are not permitted if the member previously received belimumab for SLE.

Continuation Therapy

Covered when ALL of the following are met:

Continued therapy: Member is currently receiving belimumab via the Centene benefit or previously met initial approval criteria; member is responding positively to therapy (for SLE: clinical response; for LN: reduced proteinuria with UPCR < 0.5 mg/mg from baseline OR no reduction in eGFR > 20% from baseline OR eGFR ≥ 60 mL/min/1.73 m2) with low‑dose steroids as applicable; prescribed in combination with standard SLE therapy; belimumab is not prescribed concurrently with Lupkynis (voclosporin) or another biologic agent; dosing remains within continued therapy dose limits (IV and SC maximums not exceeded).

Dose limits and administration

Dose limits (must be met):

IV dosing limits: IV dose does not exceed 10 mg/kg per dose given at 2-week intervals for the first 3 doses and every 4 weeks thereafter; for continued therapy new IV dose does not exceed 10 mg/kg every 4 weeks.

SC dosing limits - SLE: For SLE: pediatric members weighing ≥ 40 kg and adults: 200 mg once weekly; pediatric members weighing 15 to < 40 kg: 200 mg once every 2 weeks. Transition from IV to SC for SLE: first SC dose may be administered 1 to 4 weeks after the last IV dose. Maximum SC dose (outside LN loading where specified) = 200 mg per week.

SC dosing limits - LN: For LN: pediatric members weighing ≥ 40 kg and adults: 400 mg once weekly for the first 4 doses (loading), then 200 mg once weekly; pediatric members weighing 15 to < 40 kg: 200 mg once weekly for the first 4 doses then 200 mg once every 2 weeks. Transition from IV to SC for LN: after at least 2 IV doses; first SC dose 1 to 2 weeks after the last IV dose. Maximum SC dose = 200 mg/week outside the specified LN loading dosing period.

Coverage criteria (high-level)

Covered when ALL of the following are met per the policy updates and FDA labeling:

Indication: Belimumab is indicated for systemic lupus erythematosus (SLE) and lupus nephritis (LN) per updated FDA approvals, including pediatric patients aged ≥ 5 years for applicable indications.

Concurrent therapy: Member is not receiving concurrent treatment with Lupkynis (voclosporin) or another biologic agent.

Dosing and formulation: Policy includes both IV and SC dosing options; SC autoinjector pediatric dosing has been added and SLE SC dosing clarified for pediatric members weighing ≥ 40 kg.

Approval duration: Initial approval duration for Medicaid and Health Insurance Marketplace (HIM) lines of business is 12 months (reflecting maintenance therapy status).12 months

Coverage is not authorized for systemic lupus erythematosus (SLE) when the member is autoantibody‑negative. The policy explicitly lists autoantibody‑negative SLE among diagnoses for which belimumab will not be authorized. Requests lacking evidence of SLE autoantibody positivity (for example, negative ANA, anti‑dsDNA, anti‑Sm, anti‑RNP, anti‑Ro/SSA, anti‑La/SSB) should be denied per the policy exclusion.

The policy also excludes concurrent use of belimumab with certain other therapies. Specifically, belimumab is not to be prescribed concurrently with Lupkynis (voclosporin) or another biologic agent (examples cited include Saphnelo and rituximab). Requests that document simultaneous treatment with these agents are outside the scope of coverage and should be managed according to the concurrent therapy exclusion.

Concurrent treatment with Lupkynis (voclosporin) or any other biologic agent is expressly excluded by this policy for all indications. The continued therapy section and the policy change log both state that belimumab is not to be prescribed concurrently with Lupkynis or another biologic, and this exclusion was added to the policy during the 2024–2025 updates.

Providers seeking authorization must ensure the member is not receiving concurrent Lupkynis or biologic therapy; documentation of concurrent use may trigger denial per the policy’s denial‑risk language and coding/reminder guidance.

The rationale for excluding autoantibody‑negative SLE is based on trial evidence reviewed in the policy appendices: only one of the five pivotal belimumab trials included autoantibody‑negative patients, and no significant treatment effect versus placebo was observed in that subgroup. Subsequent analyses demonstrated benefit predominantly in autoantibody‑positive patients, and efficacy was confirmed in the remaining trials that enrolled autoantibody‑positive subjects.

Because of the apparent lack of demonstrated efficacy in autoantibody‑negative patients, the policy designates belimumab use in that subgroup as not authorized and therefore not medically necessary under the coverage criteria.

Uses that conflict with the policy exclusions or the specified criteria will not meet coverage requirements. Examples include: prescribing belimumab for autoantibody‑negative SLE, initiating or continuing belimumab in a member who is receiving concurrent Lupkynis (voclosporin) or another biologic agent, or requesting dosing or administration outside the listed IV/SC dose limits. Such requests are subject to denial as not authorized or not medically necessary.

Requests for non‑FDA approved indications without supporting off‑label evidence per the referenced off‑label use policies (e.g., CP.CPA.09, HIM.PA.154, CP PMN.53) or lacking required documentation (specialist consult, labs, biopsy results where applicable) also do not meet policy criteria and may be denied.
