Clinical and Administrative Context

The Wireless Motility Capsule (WMC) is an orally ingested, nondigestible data‑recording capsule that continuously measures temperature, pH, and pressure as it traverses the gastrointestinal tract to assess regional and whole‑gut transit and motility. The device is approximately 26 x 13 mm with a battery life of up to five days and is worn with an external receiver that records telemetry after the patient swallows the capsule following a standard meal. Measurements used to define transit events include changes in pH and temperature and pressure tracing: gastric emptying is identified by the capsule leaving the stomach (often associated with activity of the Phase III migrating motor complex) and the established cutoff for prolonged gastric emptying time is 300 minutes. Small bowel entry is inferred by a sharp change in pH on entry into the duodenum, and ileocecal passage is typically marked by a fall in pH at the ileocecal junction; however, in about 15% of patients this ileocecal pH drop is not observed.

Advantages of the WMC include that it is wireless, painless, and radiation‑free, and it provides simultaneous regional and whole‑gut transit data. Disadvantages and limitations include potential failure to capture usable data requiring repeat testing, delayed or failed passage of the capsule necessitating serial radiographs or endoscopic/surgical removal, and contraindications to use in patients with known or suspected strictures, altered anatomy, or severe pyloric stenosis. Patients should also be able to tolerate withholding proton pump inhibitors and H2 blockers for the test when required.

Society guidance acknowledges that WMC is generally comparable in accuracy to conventional modalities for detection of slow‑transit constipation and gastric emptying delay, and it is considered a viable alternative diagnostic modality; however, major societies note that evidence is limited regarding optimal timing in diagnostic algorithms and whether WMC adds clear clinical value beyond existing tests.
