Added Voquezna Triple/Dual Pak and Voquezna (vonoprazan) with criteria for erosive esophagitis and non-erosive GERD.
For H. pylori infection, removed required redirection to generic Prevpac for all agents except Omeclamox-Pak and added clarithromycin-sensitivity requirements for clarithromycin-containing vonoprazan regimens.
Updated contraindications for Omeclamox-Pak to include coadministration with lurasidone and simplified Talicia dosing in Section V.
