Prescriber specialty requirement: Medication must be prescribed by or in consultation with a gastroenterologist, hepatologist, infectious disease specialist, or a provider with certified HCV treatment training (unless state-specific exceptions apply; e.g., HIM Georgia exception).
Genotype and RAS testing: Provide documentation of HCV genotype; for genotype 3 patients meeting specified scenarios, provide RAS testing for velpatasvir-associated Y93H when indicated.
