Modifier Use and Coverage Criteria

Modifier use requirements and conditions of coverage

Covered when ALL of the following conditions are met:

Provider must use the most specific X{EPSU} modifier when appropriate; use modifier 59 only if no more descriptive modifier exists.

Modifier X{EPSU} (or 59, when applicable) may only be used to indicate that a distinct procedural service was performed independent from other non‑E/M services on the same day when no other more appropriate modifier is available.

Documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury not ordinarily encountered or performed on the same day by the same provider, provider group, and/or provider specialty.

XU (or 59, when applicable) may be used for a diagnostic performed after a therapeutic procedure only when the diagnostic is not a common or necessary follow‑up, occurs after completion and is not mingled with required therapeutic services, and is not otherwise included as part of the procedure.
