Reimbursement Criteria

Reimbursement criteria for JW drug waste

CareSource will consider reimbursement for discarded portions only under these conditions:

The drug must be supplied only in a single-dose or single-use vial; providers must not use the JW modifier for medications manufactured in a multi-dose vial format.

The discarded (wasted) portion must be billed on a separate claim line with the JW modifier appended; the administered dose must be billed on its own claim line without JW.

The wasted amount must represent drug not administered to another patient (i.e., the amount discarded after preparing the administered dose).

Providers must select the most appropriate vial size(s) to minimize waste; waste billed when a more appropriate vial size or combination was available is not reimbursable.

Billing procedure

Bill the administered dose on one claim line and bill the discarded (wasted) amount on a separate claim line with the JW modifier appended to the discarded line to indicate the amount wasted.

Claim Line #1: HCPCS code for drug administered with the amount actually administered to the patient.
Claim Line #2: HCPCS code for drug discarded (wasted) with the JW modifier appended and the amount discarded (wasted).
Do not bill vial overfill or package overfill; claims considered for reimbursement must not exceed the package/vial size used to prepare the dose.
Do not use the JW modifier for medications manufactured in a multidose vial format.
Do not use the JW modifier when the actual dose administered is less than the billing unit; JW applies only to the discarded amount.

Payment conditions

Reimbursement is subject to member eligibility on the date of service, medical necessity, plan benefits and limitations, provider contractual agreement, claims editing logic, applicable referral/authorization/notification requirements, and utilization management guidelines.

Services must meet medical necessity criteria and applicable federal/state mandates, Evidence of Coverage, Medical Policy Statements, Provider Manuals, and Member Handbooks.
This Policy does not guarantee authorization or reimbursement; refer to the member's plan contract (Evidence of Coverage) for benefit determinations.
CareSource may apply claim edits and provider contract terms; if there is a conflict between this Policy and the plan contract, the plan contract governs.
Coverage for behavioral health follows MHPAEA parity rules and will not be subject to less favorable limitations than medical/surgical benefits.
