Medical Necessity Criteria

Trigger Point Injections - Initial Injection (Medically Necessary Criteria)

Trigger point injections of anesthetic and/or corticosteroid for back pain, neck pain, or myofascial pain syndrome are considered medically necessary when pain has persisted despite appropriate medical management and ALL the following criteria are met:

ALL of the following

Member presents with new (acute or subacute) localized pain, occurring in the last 3 months.Timeframe: last 3 months
ALL of the following
- ONE of
  - bed rest
  - active exercise
  - ultrasound
  - range of motion
  - heating or cooling treatments
  - massage
ALL of the following
- ONE of
  - physical therapy
  - occupational therapy
  - physician supervised home exercise program (HEP)
  - manipulative therapy
ALL of the following
- ONE of
  - non-steroidal anti-inflammatory drugs (NSAIDS)
  - muscle relaxants
  - non-narcotic analgesics
  - anti-depressants
ALL of the following
- proper evaluation including a patient history and physical examination leading to diagnosis of the trigger point
- reason or reasons for selecting this therapeutic option
- affected muscle or muscles
- muscle or muscles injected and the number of injections
- frequency of injections required
- name of the medication used in the injection

Trigger Point Injections - Subsequent Injections

Criteria and limits for repeating trigger point injections:

ALL of the following

Trigger point injections should be repeated only if reasonable and medically necessary.
No more than 8 dates of service per calendar year per member may be reimbursed.<= 8 dates/year

Injections may be repeated only with documentation supporting a positive result at the last trigger point injection involving the anatomic site, including at least 50% improvement in pain, functioning, and activity tolerance.>= 50% improvement

Imaging/localization and dry needling exclusions

Claims for localization imaging associated with trigger point injections and claims for dry needling as an alternative to TPIs will not be covered.

Localization techniques to image or otherwise identify trigger point anatomic locations are not indicated and will not be covered when associated with TPIs (see Not Indicated / Not Covered).
Dry needling is not an acceptable alternative to trigger point injections by an appropriately licensed clinician and will not be covered.

Background

Background: Myofascial trigger points are self-sustaining, hyper‑irritable foci in skeletal muscle that produce referred pain patterns and comprise single‑muscle myofascial pain syndromes. The purpose of a trigger point injection (TPI) is to treat both symptom and cause by injecting a local anesthetic alone or with corticosteroid into the affected muscle to alleviate inflammation and pain.

Rationale and role: Interventional procedures, including TPIs, should be part of a comprehensive pain management plan that incorporates an initial trial of conservative treatments (eg, medications, physical therapy modalities, behavioral support) and are intended for management of pain unresponsive to conservative care.

Practice requirements: TPIs should follow a trial of conservative therapies (for example, at least 1 month of conservative measures and presentation of new localized pain within the last 3 months when applicable) and must be performed only by qualified healthcare providers acting within their scope of practice. Documentation must include evaluation/history and physical exam, reason for TPI selection, affected muscle(s), muscles injected and number, injection frequency, medication name, prior treatment results, and corroborating evidence of medical necessity.
