Medical Necessity and Coverage Criteria

Open SIJ fusion — covered indications

CareSource considers open sacroiliac joint (SIJ) fusion medically necessary when ALL the following criteria are met:

Open SIJ fusion covered indications: Procedure is being done for one of: post-traumatic SIJ injury (eg, following pelvic ring fracture); management of sacral tumor (eg, partial sacrectomy); adjunctive treatment for SIJ infection (eg, osteomyelitis, pyogenic sacroiliitis)/sepsis; part of a multisegmental long fusion construct for correction of spinal deformity (eg, idiopathic or neuromuscular scoliosis); or prior percutaneous SIJ fusion has failed.

Percutaneous (minimally invasive) SIJ fusion — covered criteria

CareSource considers percutaneous SIJ fusion/stabilization medically necessary when ALL the following are met:

Clinical localization and pain: Presence of non-radiating pain caudal to the lumbar spine (L5) localized over the posterior SIJ and consistent with SIJ pain; localized tenderness to palpation of the posterior SIJ in the absence of similar tenderness elsewhere; SIJ pain severity >= 5/10.

Provocative testing: Positive provocative testing consistent with SIJ source (eg, at least two positive maneuvers such as Gaenslen's, distraction, thigh thrust/compression, Patrick's/FABER).

Conservative therapy: Failure of conservative therapy demonstrated by documentation of at least 6 months of active conservative therapy within the past 12 months (or documented inability to complete the HEP), with additional documentation of inactive conservative therapies as appropriate.

Diagnostic blocks: Diagnostic confirmation with image-guided, contrast-enhanced intra-articular SIJ block producing >=50% pain reduction for the expected duration of the anesthetic on two separate occasions.

Imaging and exclusion of other causes: Recent (within 6 months) plain radiographs and/or CT or MRI demonstrating localized SIJ pathology; plain radiographs of the ipsilateral hip and lumbar spine CT/MRI as needed to exclude hip osteoarthritis, neural compression, destructive lesions, or inflammatory arthropathy not addressed by percutaneous fusion.

Therapeutic injection trial: Failure of a trial of at least two therapeutic intra-articular SIJ injections (eg, corticosteroid) prior to consideration of fusion.

Open or percutaneous sacroiliac joint (SIJ) fusion is not medically necessary when used for indications that are not specifically listed as covered in this policy. Examples of such excluded indications include mechanical low back syndrome, sacroiliac joint syndrome, and degenerative sacroiliac joint disease. The procedure is also not indicated for conditions driven by systemic arthropathy (for example, ankylosing spondylitis or rheumatoid arthritis), isolated infection, tumor, or fracture when not part of a covered open indication, acute traumatic SIJ instability, or when generalized pain disorders (for example, fibromyalgia or somatoform disorder) are the primary etiology of symptoms.

Percutaneous (minimally invasive) SIJ fusion is not medically necessary for indications outside those explicitly outlined as covered. Specific exclusions include SIJ pain attributable to systemic arthropathy (eg, ankylosing spondylitis, rheumatoid arthritis), infection, tumor, or fracture as a primary indication, acute traumatic SIJ instability, generalized pain disorders (eg, fibromyalgia or somatoform disorder), and when there is neural compression on lumbar MRI/CT that correlates with the patient’s symptoms or identifies a more likely pain source.

Prior authorization is required for percutaneous sacroiliac (SIJ) fusion. The prior authorization request must include complete documentation that all medical necessity criteria in this policy are met.

Include imaging reports (plain radiographs and CT or MRI of the SIJ; ipsilateral hip radiographs; lumbar spine CT/MRI if indicated).
Provide documentation of at least 6 months of conservative therapy (active and/or inactive) within the prior 12 months or documentation of inability to complete active conservative therapy due to contraindication, increased pain, or intolerance.
Include records of physical exam findings (localized posterior SIJ pain, positive provocative tests) and pain scores (SIJ pain ≥ 5/10).
Attach records confirming diagnostic confirmation via two image-guided, contrast-enhanced intra-articular SIJ blocks showing ≥50% pain reduction for the expected anesthetic duration.
Include documentation of at least two therapeutic intra-articular SIJ injections (eg, corticosteroid) and their outcomes.
Provide operative reports for prior percutaneous SIJ fusion if asserting prior fusion failure.
If applicable, document inability to complete a home exercise program (HEP) with a stated physical reason (eg, increased pain, physical inability); noncompliance alone is insufficient.

Background and Epidemiology

The sacroiliac joints connect the sacrum and the iliac bones and permit only limited normal motion (rotation and translation). SIJ dysfunction is a recognized source of low back and buttock pain and is estimated to account for approximately 15–30% of patients with low back pain. Etiologies include prior pelvic or lumbar fusion, trauma, pregnancy-related changes, inflammatory or degenerative conditions, and other structural injury. Diagnosis typically relies on focal localization of pain over the posterior SIJ, reproducible provocative maneuvers (for example, Gaenslen’s, distraction, thigh-thrust or Patrick’s tests), confirmatory image-guided intra-articular diagnostic blocks with a response of ≥50% pain reduction on two separate occasions, and imaging to exclude alternative causes; conservative and less-invasive therapies are generally attempted before considering fusion.
