- If conventional testing negative: MDT by PCR testing is medically necessary to confirm diagnosis if the member's presenting symptoms support the diagnosis.
CareSource requires that conventional testing (for example, rapid antigen direct tests, direct fluorescent antibody testing, and cultures) be used as the preferred, lower-cost initial diagnostic approach before performing molecular diagnostic testing (MDT) by PCR. Providers should perform and document conventional test results prior to ordering PCR-based MDT except in recognized exceptions (see policy).
When conventional testing is positive, additional MDT by PCR is not medically necessary. PCR-based MDT is reserved for situations in which conventional testing is negative and the member's presenting symptoms support the diagnosis.
