Coverage criteria: Acceptable medical necessity reasons include, but are not limited to, risk of aspiration and associated complications.

Products chosen for convenience or provider/individual preference are not covered.

Safety advisory: Commercial thickeners that contain xanthan gum should not be used in preterm infants because of a possible association with necrotizing enterocolitis. Avoid prescribing or supplying xanthan‑gum thickeners for members born preterm.

Use of a commercial thickener based solely on the convenience or preference of the individual or the provider does not constitute medical necessity. Coverage requires documentation of acceptable clinical reasons and practitioner attestation that the product is part of the treatment plan.
