Pediatric and recurrent UTI indications

Febrile infants and young children: Infants 2–36 months with fever >39°C and no other source should have urinalysis and, if abnormal, urine collected by catheterization or suprapubic aspiration for culture; toilet-trained children may provide midstream specimens.

CPS and AAP recommend specimen collection prior to antibiotics and specific collection methods (catheterization or SPA) for accurate diagnosis.

Recurrent symptomatic UTI: In recurrent uncomplicated UTI, obtain urine culture during the symptomatic episode and consider a post-treatment culture (eg, two weeks after initiating therapy) to confirm eradication.

CUA and joint urology guidelines recommend culture while symptomatic and follow-up culture in recurrent UTI management.

Primary indications: Urine culture is appropriate for patients with signs or symptoms localizing to the urinary tract or systemic infection (eg, fever) and for specific populations such as pregnant individuals or patients undergoing genitourinary instrumentation where mucosal bleeding is expected.

Urine culture testing (including isolate identification and antibiotic susceptibilities when applicable) is covered when one of several specific clinical contexts is present. Covered indications include: screening in pregnant individuals, testing prior to urologic procedures that will breach the mucosa, evaluation of patients with at least one sign or symptom of possible UTI or bacteriuria (see Note 1), and assessment of suspected pyelonephritis. By contrast, urine culture for most other asymptomatic contexts (including initial screening for asymptomatic prostatitis and as part of prostate biopsy assessment/prognosis) does not meet coverage criteria. The IDSA specifically recommends against screening or treating ASB in many populations (eg, most nonpregnant adults, older community-dwelling persons, long-term care residents, patients with diabetes or spinal cord injury, and many transplant recipients), while Choosing Wisely Canada similarly advises against collecting urine cultures from adults without urinary-localizing symptoms or fever except in pregnancy or when genitourinary instrumentation with expected mucosal bleeding is planned.

Routine urine cultures in asymptomatic adults are considered low-value and are discouraged by guideline and stewardship organizations. Choosing Wisely Canada and SHEA note that testing in patients without signs or symptoms produces false positives, leads to unnecessary antibiotic exposure, and should generally be avoided unless a guideline-supported exception applies.

There are clear exceptions in which surveillance cultures and treatment of ASB are appropriate. Pregnancy is a primary exception: screening and treatment of ASB using urine culture in pregnant persons meets coverage criteria. Additionally, patients undergoing urologic procedures that breach the mucosa should be screened and treated for ASB prior to the procedure to reduce infection risk, per guideline recommendations.

Surveillance cultures and treatment of ASB are not recommended for asymptomatic individuals who are nonpregnant and are not undergoing planned mucosa‑breaching procedures. The policy explicitly states that urine culture testing for other instances of asymptomatic bacteriuria does not meet coverage criteria, reflecting consensus guidance that routine surveillance leads to harm from unnecessary antibiotics and has limited benefit.

Ordering a urine culture solely for screening in adults who lack localizing urinary symptoms or fever is not medically necessary except in the specified exceptions of pregnancy or planned genitourinary instrumentation with expected mucosal bleeding. Requests for asymptomatic testing outside those exceptions are subject to noncoverage and potential denial.
