Bladder cancer is a common malignancy of the urinary tract with tens of thousands of new U.S. cases and deaths annually; it most often presents with hematuria and is predominantly diagnosed in older adults. Cystoscopy (white light) with urine cytology remains the diagnostic gold standard for identifying bladder lesions and guiding management, although it is invasive and may miss some lesions or be uncomfortable for patients.

Because cystoscopy is burdensome and costly, many urinary biomarkers have been developed to aid detection and surveillance. The best-studied commercial assays include BTA and NMP22 (available in qualitative and quantitative formats), UroVysion FISH (detects chromosomal aneuploidy and 9p21 loss), and ImmunoCyt/uCyt (fluorescent immunocytology). A growing number of proprietary and investigational tests (for example, multi-marker protein panels, methylation assays, mRNA and utDNA tests such as CxBladder, Oncuria, Bladder EpiCheck, URO17, and others) have been reported in the literature.

Published analyses and a large meta-analysis summarize substantial variability in sensitivity and specificity across biomarkers; many tests yield higher sensitivity than cytology for some settings but lower specificity, produce false positives, and often miss low-grade or low-stage tumors. As a result, guidelines generally do not endorse replacing cystoscopy with urinary biomarkers and recommend limited, adjunctive use in surveillance or to adjudicate equivocal cytology.

Cost considerations vary by test: BTA and NMP22 are relatively inexpensive (~$25), ImmunoCyt around ~$80, while multitarget or molecular tests (eg, CxBladder, UroVysion) are substantially higher (hundreds of dollars), and cystoscopy with or without biopsy has its own procedural costs—these economic factors have influenced adoption and guideline recommendations.
