Background

Radiofrequency ablation (RFA) produces thermal lesioning of nerve tissue to reduce peripheral sensory input; variations include standard RFA (thermal electrode producing high-temperature coagulative lesions), pulsed RFA (non-ablative pulses at lower temperatures), and cooled RFA (water-cooled probe creating a larger lesion). Cryoneurolysis applies very low temperatures (approximately -20°C to -100°C) to induce Wallerian degeneration leading to temporary loss of nerve function with subsequent regeneration over months. Device-specific FDA clearances exist for certain peripheral applications (for example, genicular cooled RFA probes), but the clinical evidence across indications remains heterogeneous and insufficient to demonstrate consistent long-term benefit.

Standard RFAThermal electrode delivers energy for 90–130 seconds producing tissue temperatures of about 70–90°C to create a coagulative lesion; longer-term pain relief but greater adjacent thermal tissue injury and limited lesion size/shape.

Pulsed RFANon‑ablative pulses (20 ms pulses every 30 seconds) with tissue temperature ~42°C; limits tissue damage and is thought not to destroy the nerve, with shorter duration of pain relief.

Cooled RFAWater circulates through the RF electrode to cool the tip producing a larger lesion at ~60°C with limited thermal injury to surrounding tissue; intended to provide longer‑term pain relief.
