Guideline-based testing recommendations and when HPV-specific testing or p16 IHC should be performed

Primary recommendation for oropharyngeal SCC: Perform p16 IHC as a surrogate marker to determine HPV tumor status for newly diagnosed oropharyngeal squamous cell carcinomas.

Supported by NCCN, CAP, ASCO, and EHNS/ESMO/ESTRO guidance.

When to perform additional HPV-specific testing: Perform HPV-specific testing (e.g., HR-HPV E6/E7 mRNA or other validated HPV-specific assays) in scenarios including: geographic regions with low HR-HPV prevalence; equivocal p16 staining (50%-70% or extensive but weak staining); discrepancy between p16 staining and tumor morphology; large multisite tumors overlapping the oropharynx; specimens from non-tonsillar/non-base of tongue oropharyngeal sites; when required by clinical trials; or to confirm p16-positive results in unknown primary tumors.

Recommendations derived from CAP 2025 update and other international guidelines.

Testing in squamous cell carcinoma of unknown primary (SCCUP): For SCCUP, routinely test level II and III cervical lymph node metastases for high-risk HPV; consider EBV testing for HPV-negative metastases or when clinically indicated.

Routine use of salivary biomarker tests, HPV genotyping, gene expression profiling, combined genetic/non-genetic panels, and HPV detection from oropharyngeal swabs for the purposes of screening, detection, or diagnosis DOES NOT MEET COVERAGE CRITERIA. The policy lists these modalities specifically (including salivary peptides/proteins, nucleic acids, metabolites; genotyping such as OraRisk® HPV; gene expression profiling; panels such as mRNA CancerDetect™; and oropharyngeal swab HPV PCR tests) as not supported by available published literature for diagnosis or treatment benefit.

The NCCN clinical practice guidelines for head and neck cancers do not reference adjunctive screening aids based on autofluorescence or tissue reflectance as a recommended management tool. NCCN also notes that there are no diagnostic tests with regulatory approval for HPV-related screening in the oropharynx and endorses use of p16 IHC as a widely available surrogate biomarker for tumor HPV status in oropharyngeal cancer.

Use of the listed tests for oral cancer screening, detection, or diagnosis is considered not meeting coverage criteria (i.e., not medically necessary). The policy explicitly identifies salivary biomarker testing, HPV genotyping (for example, OraRisk® HPV), gene expression profiling, combined genetic/non-genetic biomarker panels (for example, mRNA CancerDetect™), and HPV detection from oropharyngeal swabs (for example, OmniPathology Oropharyngeal HPV PCR Test) as excluded due to insufficient published evidence demonstrating clinical utility.

Adjunctive screening aids that rely on autofluorescence or tissue reflectance (for example, hand-held fluorescence devices) are not supported by NCCN as management tools for head and neck cancer screening. Because NCCN does not mention these techniques in its guidance, routine use of such adjunctive devices lacks guideline support and is not endorsed for clinical management.

Background on Oral and Oropharyngeal Cancer

Oral cancer primarily arises as squamous cell carcinoma in the oral cavity and oropharynx. Many cases are preceded by potentially malignant disorders; however, there is no approved or validated testing strategy for routine population screening of the mouth or throat. Human papillomavirus (HPV), particularly high-risk types, is a major contributor to oropharyngeal squamous cell carcinoma (OPSCC). For established oropharyngeal SCC or metastatic SCC of unknown primary in a cervical lymph node, testing to determine tumor HPV status using either high-risk HPV E6/E7 mRNA expression testing or p16 immunohistochemistry (IHC) MEETS COVERAGE CRITERIA. Conversely, many proposed noninvasive or adjunctive methods (salivary biomarkers, HPV genotyping from saliva, brush cytology, autofluorescence/tissue reflectance aids, and combined biomarker panels) lack sufficient standardized evidence to replace conventional oral examination followed by biopsy and histopathology.
