Coverage Criteria

Initial IOL Implant

Covered when ANY of the following conditions exist (standard monofocal IOL):

Indications for standard monofocal IOL: Following cataract extraction OR trauma to the eye which has damaged the lens OR congenital cataract OR congenital aphakia OR lens subluxation/displacement OR anisometropia of >= 3 diopters with uncorrectable vision with glasses or contact lensesanisometropia >= 3 diopters

Premium intraocular lens implants (presbyopia-correcting, astigmatism-correcting, phakic IOLs) are not medically necessary for any indication

Replacement of IOL

Replacement of a previously implanted IOL is medically necessary when:

Replacement indications: Anatomical change OR inflammatory response OR mechanical failure rendering a previously implanted intraocular lens ineffective or nonfunctional

Spectacle/Contact Lens Coverage for Aphakia and Pseudophakia

Coverage specifics:

Aphakia initial coverage: For congenital absence of the lens or after cataract removal without IOL insertion: first pair of glasses or aphakic contact lenses (bifocal, trifocal, separate far and near lenses, or contact lenses) are medically necessary; additions such as anti-reflective coating (V2750), tinted lenses (V2744, V2745), polycarbonate lenses for single-eye functional vision (V2784, S0580), oversize lenses (V2780), and UV protection (V2755) may be considered medically necessary

Contact lenses and near lenses may be used together or alternately

Aphakia replacement: An additional pair of lenses (eyeglass or contact) is medically necessary each time the member's prescription changes for members without an IOL; refer to the member's certificate of coverage/benefits booklet for replacement timing and limits

Pseudophakia postoperative coverage: One pair of eyeglasses or contact lenses is medically necessary after each cataract surgery with insertion of an IOL; if two surgeries occur without interim lenses, only one pair after the second surgery is covered; replacement frames or lenses follow the member's certificate of coverage

Iris Prosthesis

Iris prosthesis may be considered medically necessary for ALL of the following listed diagnoses:

Iris prosthesis indications: Aniridia OR Traumatic injury to the eye OR Albinism (Achromasia) OR Melanoma of the eye

All other indications are considered investigational

Mixed coverage — covered when medically necessary vs not medically necessary

Coverage varies by service type; some items are covered when medically necessary while others are explicitly not covered.

Covered services: Intraocular lens implants (covered when medically necessary; e.g., V2630-series) and spectacle correction and contact lenses (various V21xx/V23xx/V25xx codes) are covered when medically necessary per the member's benefit plan

Coverage determined by medical necessity and the member certificate of coverage

Not covered services: Premium intraocular lens implants (presbyopia-correcting, astigmatism-correcting, phakic IOLs), certain spectacle customization codes (specific V27xx codes), and low vision aids (V2600-series) are listed as not medically necessary and not covered

CustomFlex requirements: CustomFlex Artificial Iris (HumanOptics) is the only FDA‑approved standalone prosthetic iris; it can be sized and colored for each patient and is indicated for congenital and traumatic aniridia and other iris defects; providers must complete the manufacturer's online certification training before ordering

Iris prosthesis coverage: Iris prosthesis (e.g., C1839 mapped to procedure codes such as 66683 and 66999) is covered when medically necessary for listed diagnoses including absence of iris, traumatic iris loss/injury, ocular albinism variants, congenital aniridia/aphakia, or malignant neoplasm of the ciliary body

Diagnostic ICD‑10 codes enumerated in the policy (e.g., Q13.1, E70.x, C69.x, H27.x, S05) support medical necessity

Premium intraocular lenses (also termed premium IOLs, including presbyopia-correcting, multifocal/accommodative, astigmatism-correcting/toric, and phakic IOLs) are specified in this policy as not medically necessary for any indication. The policy notes these lenses are intended to reduce dependence on reading glasses and therefore are excluded from medical necessity coverage for aphakia, pseudophakia, or other lens-replacement indications. (See policy listing of premium IOL examples and the general statement that such devices are not medically necessary.)

Certain device and supply categories are explicitly listed as not medically necessary and therefore not covered. This includes premium intraocular lens implants (see HCPCS/CPT groupings such as C1780 and associated procedure codes), specific spectacle/eyewear feature codes (various V27xx entries), and low vision aids (V2600 series). Providers should reference the coding lists in the policy when submitting claims to avoid submission of noncovered codes.

Cosmetic (non–medically necessary) iris implants are associated with documented safety concerns. The policy cites the American Academy of Ophthalmology warning that cosmetic iris implants carry a risk of permanent eye damage and vision loss, and therefore cosmetic iris implantation is not supported by clinical safety statements and is not considered medically necessary.

For routine cataract surgery or other standard indications where the natural lens is absent, this policy supports coverage of a standard monofocal IOL but explicitly states that premium intraocular lens implants are not medically necessary. Standard monofocal IOLs remain the covered lens type when medical necessity criteria are met (e.g., following cataract extraction, trauma, congenital lens disorders, lens subluxation, or anisometropia ≥ 3 diopters with uncorrectable vision).

The policy reiterates that specific premium IOL product codes (for example C1780 and related procedure codes), a set of spectacle- and contact-related HCPCS V27xx codes, and low vision aid codes (V2600–V2615) are designated as not medically necessary and therefore not covered. Providers should not expect reimbursement for these listed items unless otherwise specified by the member’s benefit plan.

The policy emphasizes that cosmetic iris implants are linked to adverse outcomes in the literature and professional society statements. Because of the risk of permanent ocular damage and vision loss referenced by the American Academy of Ophthalmology, cosmetic iris implantation for non‑medical purposes is not supported and is not considered medically necessary.

Premium intraocular lenses are not medically necessary

Premium intraocular lens (IOL) implants intended to reduce the need for reading glasses are not medically necessary for any indication and should not be submitted as covered. Providers should not bill Capital Blue Cross for premium IOLs.

Background

Aphakia is the absence of the eye’s natural lens, which causes loss of accommodation and a hyperopic shift in refraction. Vision after aphakia can be restored by thick spectacle lenses, contact lenses, or by implantation of an intraocular lens. The policy notes that a standard monofocal IOL is the medically supported implant for covered indications, while premium IOLs are excluded.
