Coverage Criteria — Noncontraceptive IUD Use

Medically necessary indications for 52 mg LNG-IUS

Covered when ALL of the following are met for LNG IUS 52 mg (Mirena, Liletta):

FDA-labeled indication: Use of 52 mg levonorgestrel intrauterine system (LNG-IUS) devices (Mirena®, Liletta®) for treatment of heavy menstrual bleeding per FDA labeling is considered medically necessary.

Supported by FDA approvals cited in background.

Off-label but acceptable indications: Off-label use of 52 mg LNG-IUS may be considered medically necessary for any of the following: idiopathic menorrhagia OR menometrorrhagia OR as an alternative to other hormonal regimens or surgical interventions for excessive or irregular bleeding OR as endometrial protection in individuals receiving selective estrogen receptor modulators OR management of recurrent pelvic pain secondary to multi-treated endometriosis.

Policy explicitly lists these off-label indications as medically necessary for 52 mg devices.
