Background: Multimarker serum tests (MAAAs) were developed to aid assessment of adnexal masses. Three tests have been FDA-cleared for marketing: OVA1 (cleared 2009), ROMA (cleared 2011), and Overa (cleared 2016). OvaWatch was introduced in December 2022 but was not FDA approved at the time of this policy.

Evidence and guidance: Studies show these tests have moderate to high sensitivity with variable specificity (examples: OVA1 sensitivity 92% specificity 42%; Overa sensitivity 94% specificity 65%; ROMA sensitivity 90% specificity 67%). However, no randomized trials demonstrate that management guided by these tests improves health outcomes. National guidance notes that patients with suspected ovarian cancer should be evaluated by an experienced gynecologic oncologist, and multimarker tests are intended to be used in conjunction with clinical assessment rather than as stand-alone tests.
