Indications for Urine Culture

Covered when ANY of the following clinical indications are present:

Pregnancy: Pregnant individuals should be screened for asymptomatic bacteriuria using midstream urine culture (gold standard); if culture is not available, onsite midstream Gram-stain is preferred over dipstick.

WHO recommendation; culture is gold standard.

Peri-procedural / urologic procedures: Patients undergoing endoscopic or urologic procedures that breach the mucosa should have urine culture performed prior to the procedure and treated if ASB is present.

EAU and IDSA recommend screening and treating before mucosa‑breaching procedures; AUA requires culture prior to such interventions.

Suspected complicated infection or pyelonephritis: Patients with suspected complicated UTI, pyelonephritis, urosepsis, or systemic signs (e.g., fever, flank pain) should have urine culture with antimicrobial susceptibility testing; obtain cultures prior to initiating antimicrobials when possible.

AUA and EAU guidance support culture and susceptibility testing in pyelonephritis and complicated infections.

ASB Screening and Treatment Exceptions

Covered when ALL of the following are met (per IDSA and supporting societies):

ASB screening and treatment exceptions: Screening for and treatment of asymptomatic bacteriuria is recommended for pregnant individuals and for patients undergoing endoscopic urologic procedures associated with mucosal trauma; for most other populations (healthy nonpregnant adults, many older adults, patients with diabetes, renal transplant recipients >1 month post-transplant, those with short‑term indwelling catheters, etc.) screening and treatment are recommended against.

Kidney Transplant Recipients

Special management for kidney transplant recipients:

Kidney transplant (KT) recipients—ASB management: If asymptomatic bacteriuria is detected in a kidney transplant recipient, obtain a second confirmatory urine culture to minimize contamination and review results prior to deciding on treatment; observe without antibiotics if the initial bacteriuria clears or a different organism is found; avoid routine screening >2 months post‑transplant.

Per American Society of Transplantation Infectious Diseases guidance.

Routine screening urine cultures for conditions such as asymptomatic prostatitis, for assessment or prognosis surrounding prostate biopsy, and surveillance cultures for asymptomatic bacteriuria are explicitly excluded from coverage. The policy states that urine culture testing in these situations "DOES NOT MEET COVERAGE CRITERIA" due to lack of evidence that testing benefits diagnosis or treatment, and therefore such uses are not considered medically necessary.

Routine screening urine cultures in asymptomatic nonpregnant adults — including many long-term care residents, older adults, and many functionally impaired persons — are discouraged. Multiple guideline bodies recommend against screening or routine surveillance cultures in asymptomatic adults (unless pregnant or undergoing mucosa‑breaching genitourinary procedures), and laboratories are encouraged to reduce processing of such low‑value specimens.

Testing asymptomatic adults is excluded from coverage except in two specified situations: pregnancy and pre‑procedural evaluation before urologic interventions that breach mucosa. The policy aligns with guideline recommendations that urine cultures in asymptomatic adults should only be performed when these exceptions apply.

Follow‑up urine cultures after clinical resolution of an uncomplicated UTI are not covered. Likewise, surveillance cultures for asymptomatic bacteriuria are considered not medically necessary because clinical resolution alone is generally sufficient to determine treatment effectiveness and surveillance testing has not demonstrated benefit but may cause harm through unnecessary antibiotic use.

Guidance supporting the exclusion includes Choosing Wisely and pediatric recommendations: the AAP and related pediatric societies advise against surveillance cultures for asymptomatic bacteriuria and against routine follow‑up cultures after uncomplicated UTI resolution, while Choosing Wisely Canada and other societies recommend not collecting urine cultures from adults lacking urinary symptoms unless pregnant or undergoing instrumentation with expected mucosal bleeding.

Routine urine cultures in asymptomatic patients without the policy’s specified exceptions are classified as low‑value and inappropriate. Clinical guidance cited in the policy uniformly advises against screening asymptomatic adults (except pregnancy and select peri‑procedural indications) because positive cultures in low‑likelihood settings commonly lead to unnecessary treatment.
