Clinical and Administrative Context

Warfarin dosing has a narrow therapeutic window and marked interindividual variability in maintenance dose requirements. Genetic variants in CYP2C9 and VKORC1 (and to a smaller extent CYP4F2) influence warfarin metabolism and dose requirements; CYP2C9*2 reduces metabolism by ≈30% and CYP2C9*3 by ≈90%, and CYP4F2 variants account for a smaller proportion of dose variability. Algorithms integrating these genotypes with clinical factors have been developed to predict starting and maintenance doses, but no single algorithm is universally accepted and variant frequencies differ across racial and ethnic groups.

Multiple randomized controlled trials and systematic reviews have evaluated genotype-guided warfarin dosing. Pooled evidence shows genotype-guided approaches can increase the percentage of time patients spend in the therapeutic INR range in some studies and meta-analyses, and some pooled analyses suggest reduced major bleeding. However, trials are heterogeneous, often small or underpowered for clinical endpoints such as major bleeding, thromboembolic events, or mortality, and results are inconsistent across populations. Because of these limitations, the evidence is insufficient to conclude a consistent net health outcome benefit from routine genotype-guided warfarin dosing.

Several assays that detect these variants have been cleared by the U.S. Food and Drug Administration for marketing; these tests are intended to be used with clinical evaluation and INR monitoring rather than as stand-alone determinants of dosing. Genotype information is included in warfarin (Coumadin) labeling, but testing is not mandatory per FDA labeling.
