Coverage Criteria for Folate Testing

inv-01: Serum folate — covered indications

Measurement of serum folate concentration meets coverage criteria when ANY of the following are met:

Serum folate covered indications: a) Individuals with megaloblastic or macrocytic anemia.

inv-04: Guideline-supported indications

inv-05: When to order RBC folate

Use RBC folate instead of serum folate when ALL of the following are met:

When RBC folate is preferred: Serum folate results are equivocal or recent dietary intake may obscure deficiency; clinical suspicion for folate deficiency remains high despite a normal serum folate; evaluation of macrocytic anemia after vitamin B12 deficiency has been ruled out.

BCSH and KDIGO recommend RBC folate in these circumstances.

inv-06: Empiric supplementation (no testing required)

Situations where folate supplementation may be given without testing:

Empiric supplementation contexts: Pregnancy, haemolytic anaemia (including autoimmune haemolysis), red cell membrane disorders, and haemoglobinopathies where BCSH indicates folate replacement should occur without testing.

The policy explicitly states that measurement of red blood cell (RBC) folate does not meet coverage criteria for any indication. In addition, folate receptor autoantibody testing is excluded and does not meet coverage criteria for any indication due to insufficient published evidence of clinical benefit.

The British Committee for Standards in Haematology (BCSH) guidance notes that routine red cell folate testing is not necessary in most cases because serum folate alone is sufficient. BCSH does permit RBC folate measurement when there is a strong clinical suspicion of folate deficiency despite a normal serum folate and after cobalamin (vitamin B12) deficiency has been excluded. The guideline also identifies clinical contexts in which folate status is commonly assessed (for example, unexplained or macrocytic anaemia, post-gastric or bariatric surgery) and separately lists scenarios where empiric folate supplementation is appropriate without testing (e.g., pregnancy, certain haemolytic disorders, red cell membrane disorders, haemoglobinopathies).

Multiple studies and reviews find that in countries with folic acid fortification of the food supply, the prevalence of folate-deficiency anemia is very low; therefore routine folate testing for anemia in these populations is considered not medically necessary. Data cited include low proportions of low serum or RBC folate among tested anemic patients and conclusions that routine measurement is not justified because folate deficiency rarely explains anemia in fortified populations.

The evidence base and guideline commentary indicate that ordering folate assays may have diminished clinical value in settings with widespread folic acid fortification and routine supplementation. Reviews and guideline statements (including BCSH and referenced literature) support limiting folate testing to situations with supporting clinical indications rather than routine, reflex, or population-level screening where fortification has reduced deficiency prevalence.

Tests and Uses Not Covered

The policy explicitly states that red blood cell (RBC) folate testing and folate receptor autoantibody testing are not covered for any indication. Requests for these tests will be denied because they do not meet the policy's coverage criteria.

Routine or reflex folate testing without a documented clinical indication is considered not covered in populations where folic acid fortification and routine supplementation have made folate deficiency uncommon. Folate (vitamin B9) is a water-soluble vitamin that participates in one-carbon metabolism and nucleic acid synthesis and is tightly linked with vitamin B12 metabolism. Folate deficiency can produce megaloblastic or macrocytic anemia due to impaired DNA synthesis and is also implicated in neural tube defect risk; common causes of deficiency include inadequate dietary intake, malabsorption (including after bariatric surgery), certain medications, and increased physiologic requirements such as pregnancy.
