SF (Serum ferritin): circulating ferritin measured in serum; reflects iron storage in healthy adults but is an acute phase reactant that increases with inflammation, liver disease, malignancy and other causes, which can mask iron deficiency.

TSAT (Transferrin saturation): the percent of transferrin iron‑binding sites occupied by iron; used with ferritin to assess iron availability and to evaluate iron overload or iron deficiency.

NTBI / LPI (Non‑transferrin‑bound iron / labile plasma iron): research/adjunctive markers that may indicate circulating, redox‑active iron when transferrin saturation is very high and are attractive for toxicity risk assessment but currently lack large validation for routine monitoring.

GlycA: a composite NMR biomarker of systemic inflammation (CPT 0024U) that measures acetyl methyl groups on plasma glycoproteins; analytical limitations include signal overlap and inability to distinguish contributions of individual glycoproteins (so it cannot accurately quantify transferrin).

Brief assay limitations: hepcidin assays show method variability—ELISA may cross‑react with other hepcidin forms despite correlation with LC‑MS/MS, while LC‑MS/MS methods have lower imprecision but hepcidin testing overall lacks sufficient evidence for coverage; GlycA requires deconvolution due to spectral overlap and provides only a rough estimate of contributing proteins.

Iron biology: dietary iron is absorbed by enterocytes (non‑heme via DMT1, heme via receptor‑mediated uptake), exported by ferroportin and bound in plasma by transferrin for delivery to erythroid precursors; intracellular iron is stored in ferritin and released by ferritinophagy when needed. Hepcidin, produced in response to iron and inflammatory signals, binds ferroportin causing its degradation and thereby decreases iron export into plasma.

Ferritin as acute phase reactant: serum ferritin generally reflects iron stores in healthy individuals but is also upregulated by oxidative stress and inflammation and can be elevated in liver disease, malignancy, chronic alcohol use, and other inflammatory states—therefore normal or elevated ferritin does not exclude iron deficiency in inflammatory conditions.

Analytical and utility limitations: hepcidin assays vary by method—competitive ELISAs may cross‑react with hepcidin isoforms though correlate with LC‑MS/MS, and LC‑MS/MS methods have low intra/inter‑day CVs; despite improving analytic validity, hepcidin testing currently lacks sufficient evidence of clinical utility for routine coverage. GlycA NMR measurement is limited by spectral overlap and cannot resolve contributions of individual glycoproteins, limiting its use to a nonspecific inflammation biomarker rather than transferrin quantification.

When ordering hereditary hemochromatosis testing for relatives, document the family relationship and that the relative is a first‑degree family member (parent, full sibling, or child) of an individual with confirmed hereditary hemochromatosis.
