For relapsing fevers and babesiosis/HTRF/STRF, whole blood is preferred for NAAT and microscopy (dark‑field or Giemsa/Wright) during febrile episodes. For malaria, perform STAT Giemsa‑stained thick and thin smears (repeat every 12–24 hours up to 3 exams before ruling out) and consider RDTs as an alternative when microscopy is not immediately available; use PCR/NAAT for species confirmation. For neuroborreliosis and other CNS presentations, collect paired CSF and serum (ideally within 24 hours) for antibody testing or CSF/serum index evaluation. For Zika and other arboviruses, follow specific windows: Zika NAAT on serum/urine/whole blood/CSF for pregnant symptomatic individuals up to 12 weeks after symptom onset or travel, and Zika/DENV NAAT on serum collected ≤ 7 days after symptom onset for symptomatic non‑pregnant individuals; perform PRNT to resolve cross‑reactive flavivirus IgM positives. Asymptomatic screening denial risk

Asymptomatic screening for vector-borne infections during a general exam without abnormal findings is a denial risk. Do not order routine screening tests for babesiosis, chikungunya, Colorado tick fever, dengue, ehrlichiosis/anaplasmosis, malaria, rickettsial disease, tickborne relapsing fever, West Nile, yellow fever, or Zika in asymptomatic individuals without exposure or clinical signs.
