Coverage Criteria for Radiofrequency Ablation (Non-Liver Solid Tumors)

Initial therapy — covered indications

Covered when ALL of the following are met for each indication category:

Painful osteolytic bone metastases — palliative RFA: Patient has painful osteolytic bone metastases AND has failed or is a poor candidate for standard treatments such as radiation or opioids.

Evidence shows clinically significant pain relief and reduced opioid use in this population.

Osteoid osteoma that cannot be managed successfully with medical treatment.

High clinical success rates (pain-free 94%-98%) reported in observational studies.

Localized renal cell carcinoma ≤ 4 cm AND (either to preserve kidney function in patients with significant renal impairment (one kidney or GFR < 60 mL/min/m2) when standard surgery would substantially worsen kidney function OR the patient is not a surgical candidate).≤ 4 cm; GFR < 60 mL/min/m2 as renal insufficiency

Evidence is mixed versus partial nephrectomy; RFA may be used to preserve renal function or when surgery is not feasible.

Isolated peripheral non-small cell lung cancer lesion: Isolated peripheral NSCLC lesion ≤ 3 cm AND surgical resection or curative-intent radiation is appropriate by stage but the patient is unfit for those interventions due to comorbidity AND the tumor is located ≥ 1 cm from the trachea, main bronchi, esophagus, aorta (and arch branches), pulmonary artery, and the heart.≤ 3 cm; ≥ 1 cm distance

Additional guidance: no more than 3 tumors per lung; tumors should be amenable to complete ablation; wait 12 months before repeat ablation.

Malignant nonpulmonary tumors metastatic to lung: Metastatic nonpulmonary tumor(s) to the lung ≤ 3 cm when (to preserve lung function because surgical resection or radiation would substantially worsen pulmonary status OR the patient is not a surgical candidate) AND there is no evidence of extrapulmonary metastases AND the tumor is located ≥ 1 cm from the specified thoracic structures.≤ 3 cm; ≥ 1 cm distance

Intended to preserve pulmonary function when other definitive therapies are contraindicated or not feasible.

Investigational / Not medically necessary

Not covered / investigational when ANY of the following apply:

Investigational indications: Breast tumors; lung cancer not meeting the specific lung criteria above; renal cell cancer not meeting the renal criteria above; osteoid osteomas manageable with medical treatment; painful bony metastases as initial treatment (i.e., before standard therapies); and all other tumors outside the liver including head and neck, adrenal gland, ovary, and unspecified pelvic/abdominal metastases.

The policy states there is insufficient evidence to support favorable health outcomes for these indications, and treatments in these categories may be denied as investigational.

Radiofrequency ablation procedures billed with CPT code 41530 are designated investigational and not covered by this policy. Use of this code for RFA of non-liver solid tumors may result in claim denial.

RFA for breast tumors is considered investigational and not medically necessary. The evidence is limited to observational studies and systematic reviews that report variable and incomplete ablation rates, concerns about residual tumor viability after ablation, and no demonstrated long-term improvements in local control or survival compared with conventional breast-conserving treatment. As a result, long-term benefit has not been established and the procedure is not covered for this indication.

Background and Rationale

Radiofrequency ablation (RFA) is a thermal ablation technique that uses alternating radiofrequency energy delivered via a probe to produce local coagulation necrosis of tumor tissue. It can be performed by percutaneous, laparoscopic, or open approaches depending on tumor location and patient factors. The primary goals of RFA are local tumor control and symptom palliation (for example, pain relief for osteolytic bone metastases), and in select circumstances to preserve organ function or provide a less morbid alternative for patients who are not surgical candidates. Complications relate to thermal injury of adjacent structures and procedure-specific risks (for example, probe-track injury or pneumothorax with thoracic access).
