Primary exclusion: Cytologic analysis of epithelial cells to assess breast cancer risk and manage patients at high risk of breast cancer DOES NOT MEET COVERAGE CRITERIA.

Step Therapy

Preferred diagnostic modality — prefer core needle biopsy over FNA

For tissue diagnosis of a suspicious breast mass, core needle biopsy is preferred over fine needle aspiration because core biopsy has superior sensitivity, specificity, and more accurate histologic grading.

Background

Nipple aspiration and ductal lavage are techniques used to obtain breast epithelial cells for cytologic examination. These procedures collect cellular material from the nipple/ductal system to evaluate nipple discharge and to attempt detection of epithelial atypia; however, the policy notes there is insufficient evidence that such cytologic testing provides the necessary diagnostic or prognostic benefit to support its use for breast cancer risk assessment or routine management of high‑risk patients.
