Clinical Background

Lipoprotein(a) (Lp(a)) is an independent but overall modest risk factor for cardiovascular disease; population Lp(a) levels are largely genetically determined and can vary widely between individuals, and a single-nucleotide variant in the LPA gene, rs3798220 (I4399M), has been associated with elevated Lp(a) levels and an increased risk of cardiovascular events.

The rs3798220 allele is infrequent in the population but has been reported in observational studies and Mendelian randomization analyses to be causally linked to higher Lp(a) and greater cardiovascular risk. Because carriers may have higher thrombosis risk, some observational and post hoc analyses (for example, analyses from the Women's Health Study) have suggested that carriers might derive greater benefit from aspirin than noncarriers.

However, testing performance characteristics and standardization of LPA variant testing are uncertain, studies have been inconsistent, and it is unclear whether providing genetic information about rs3798220 leads to meaningful changes in clinical management or improved health outcomes. In particular, available evidence does not establish that deviating from current guideline-based aspirin recommendations based on LPA genotype improves patient outcomes.
