RFA is considered medically necessary when ALL of the following are met:

Investigational / Not Medically Necessary

RFA is considered investigational under the following conditions:

ANY of the following

Primary, inoperable HCC when sites of tumor foci cannot be adequately treated
When used to downstage (downsize) hepatocellular carcinoma in individuals being considered for liver transplant
Primary, operable hepatocellular carcinoma (i.e., operable HCC)
Hepatic metastases from colorectal cancer or neuroendocrine tumors that do not meet the above criteria
Hepatic metastases from other types of cancer (non-colorectal, non-neuroendocrine)

Background & Evidence

Radiofrequency ablation (RFA) is a locoregional thermal ablation technique in which a probe or needle electrode is inserted into a tumor percutaneously, laparoscopically, or via open approach and heated by high-frequency alternating current to produce a roughly 3–5 cm sphere of coagulative necrosis. The ablated tissue undergoes fibrosis and scar formation; local recurrences, when they occur, typically arise at the treatment margin and may be retreated.

Evidence supports RFA as medically necessary for inoperable HCC meeting Milan criteria and as a bridge to liver transplantation, and for selected inoperable colorectal and neuroendocrine liver metastases when policy conditions are met. By contrast, the evidence is insufficient to support RFA for many other metastatic tumor origins and for operable HCC, where resection generally has superior outcomes.

Definitions

Milan criteria: single tumor ≤ 5 cm or up to 3 foci each ≤ 3 cm, with absence of angioinvasion and absence of extrahepatic involvement (used to determine transplant eligibility and applicable RFA indications).

Inoperable: Tumors not amenable to surgical resection because of location, size, number, or comorbid conditions that preclude surgery (as used in the policy context).

Bridge to transplant: Use of locoregional therapy, including RFA, to prevent tumor progression and maintain an individual's candidacy for liver transplantation while awaiting transplant.
